摘要
目的 观察氨茶碱联合无创呼吸机对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者肺功能及动脉血气指标的影响。方法 选择2020年6月—2022年6月石家庄市人民医院收治的COPD合并呼吸衰竭患者70例,采用随机数字表法分为试验组和对照组,每组35例。对照组采用预防感染、祛痰、解痉等基础对症治疗及无创呼吸机辅助治疗,试验组在对照组基础上联合氨茶碱治疗,2组患者均治疗5 d。比较2组治疗前后肺通气功能[第1秒用力呼气容积(FEV_(1))占预测值百分比(FEV_(1)%)、FEV_(1)与用力肺活量(FVC)比值(FEV_(1)/FVC)及呼气流量峰值(PEF)]、动脉血气指标[pH值、动脉血氧分压(PaO_(2))及动脉血二氧化碳分压(PaCO_(2))],ICU住院时间、总住院时间及住院病死率。结果 治疗5 d后,2组FEV_(1)%、FEV_(1)/FVC、PEF及PaO_(2)均高于治疗前,PaCO_(2)低于治疗前,且试验组变化幅度大于对照组(P<0.01);2组pH值与治疗前比较均无统计学意义(P>0.05)。试验组ICU住院时间、总住院时间短于对照组(P<0.01);2组住院病死率比较差异无统计学意义(P>0.05)。结论 氨茶碱联合无创呼吸机对COPD合并呼吸衰竭患者肺功能及动脉血气指标的改善效果较好,可促进患者快速康复。
Objective To observe the effect of aminophylline combined with non-invasive ventilator on lung function and arterial blood gas in patients with COPD combined with respiratory failure.Methods A total of 70 patients with COPD combined with respiratory failure admitted to Shijiazhuang People′s Hospital from June 2020 to June 2022 were selected,and they were divided into experimental group and control group according to the randomized numerical table method,35 cases in each group.The control group was treated with basic symptomatic treatment such as preventing infection,expectorant,antispasmodic and non-invasive ventilator-assisted treatment,and the experimental group was treated with aminophylline on the basis of the control group,and the two groups were treated for 5 days.The pulmonary ventilation function indexes(FEV_(1)%,FEV_(1)/FVC and PEF)and arterial blood gas indexes(pH value,PaO_(2),PaCO_(2))before and after treatment,ICU hospitalization time,total hospitalization time and hospitalization mortality rate were compared between the two groups.Results After 5 days of treatment,FEV_(1)%,FEV_(1)/FVC,PEF and PaO_(2) of the two groups were higher than those before treatment,and PaCO_(2) was lower than that before treatment,and the magnitude of change in experimental group were greater than those of control group(P<0.01);pH value in the two groups did not change significantly compared with pre-treatment(P>0.05).The ICU hospitalization time and total hospitalization time of experimental group were shorter than those of control group(P<0.01).There was no statistically significant difference in the hospitalization mortality rate between the two groups(P>0.05).Conclusion Aminophylline combined with non-invasive ventilator has a good effect on improving pulmonary function and arterial blood gas in patients with COPD combined with respiratory failure.
