黄藤合剂经验方治疗放射性肠炎的临床效果

Clinical effect of Huangteng mixture experience prescription in the treatment of radiation enteritis

目的 观察黄藤合剂经验方治疗放射性肠炎的临床效果。方法 选取2021年10月—2022年9月蚌埠医学院第一附属医院收治的放射性肠炎患者60例,根据随机数字表法分为观察组和对照组,各30例。观察组予黄藤合剂加减配合中药灌肠和肚脐贴敷治疗;对照组予曲美布汀,腹泻明显加洛哌丁胺胶囊,康复新液每晚保留灌肠治疗,2组均治疗8周。比较2组治疗效果,治疗前后超氧化物歧化酶(SOD)水平、CD4^(+)/CD8^(+)、KPS评分和视觉模拟评分法(VAS)评分及主要症状积分。结果 观察组治疗总有效率为76.67%,高于对照组的50.00%(χ^(2)=4.593,P=0.032)。治疗8周后,观察组SOD水平和CD4^(+)/CD8^(+)高于治疗前及对照组(P<0.05或P<0.01),对照组SOD水平和CD4^(+)/CD8^(+)与治疗前比较差异均无统计学意义(P>0.05);2组KPS评分较治疗前升高,VAS评分较治疗前降低,且观察组升高/降低幅度大于对照组(P<0.05或P<0.01);2组腹痛、腹泻、便血积分较治疗前降低,且观察组低于对照组(P<0.05或P<0.01)。结论 黄藤合剂结合中药灌肠和肚脐贴敷治疗放射性肠炎能取得良好治疗效果,可减轻患者腹泻、腹痛、便血等不适症状,改善免疫功能,提高KPS评分,降低VAS积分。

Objective To observe the clinical effect of Huangteng mixture experience prescription in the treatment of radiation enteritis.Methods A total of 60 patients with radiation enteritis admitted to the First Affiliated Hospital of Bengbu Medical College from October 2021 to September 2022 were selected and randomly divided into observation group and control group,with 30 cases in each group.The observation group was treated with Huangteng mixture combined with traditional Chinese medicine enema and navel application,while the control group was treated with trimebutine,patients with obvious diarrhea plus loperamide capsules,and Kangfuxin Liquid retention enema every night.Both groups were treated for 8 weeks.The treatment effect,the levels of SOD,CD4^(+)/CD8^(+),KPS scores,VAS scores and main symptom scores were compared between the two groups before and after treatment.Results The total effective rate of the observation group was 76.67%,higher than 50.00%of the control group(χ^(2)=4.593,P=0.032).After 8 weeks of treatment,the level of SOD and CD4^(+)/CD8^(+)in the observation group were higher than those before treatment and the control group(P<0.05 or P<0.01),while there was no significant difference of the level of SOD and CD4^(+)/CD8^(+)in the control group before and acfter treatment(P>0.05).The KPS scores of the two groups were higher than those before treatment,and the VAS scores were lower than those before treatment,and the increase/decrease amplitude of the observation group were greater than those of the control group(P<0.05 or P<0.01).The scores of abdominal pain,diarrhea and hematochezia in the two groups were lower than those before treatment,and the observation group were lower than those in the control group(P<0.05 or P<0.01).Conclusion Huangteng mixture combined with traditional Chinese medicine enema and navel application in the treatment of radiation enteritis can achieve good therapeutic effect,which can improve the symptoms of diarrhea,abdominal pain,hematochezia and the other discomfort,improve immune function,improve KPS scores,and reduce VAS scores.
