急性心肌梗死患者并发心源性休克的影响因素分析

Influencing factors of cardiogenic shock in patients with acute myocardial infarction

目的分析急性心肌梗死患者并发心源性休克的影响因素。方法抽取2021年3月至2022年5月南阳市第一人民医院收治的急性心肌梗死患者110例,依据心源性休克的发生情况分为对照组(55例,未并发心源性休克)和观察组(55例,并发心源性休克)。收集患者临床资料(性别、年龄、发病至就诊时间、是否合并慢性病、心功能分级、是否接受溶栓治疗、吸烟史、饮酒史),比较两组临床资料,将差异有统计学意义的项目纳入Logistic回归方程进行计算,分析急性心肌梗死患者并发心源性休克的影响因素。结果两组性别、吸烟史、饮酒史比较差异未见统计学意义(P>0.05),两组年龄、发病至就诊时间、是否合并慢性病、心功能分级、是否接受溶栓治疗比较差异有统计学意义(P<0.05)。Logistic回归方程结果显示,年龄、发病至就诊时间、是否合并慢性病、心功能分级、是否接受溶栓治疗是急性心肌梗死并发心源性休克的影响因素(P<0.05)。结论急性心肌梗死患者并发心源性休克的影响因素主要包括年龄、发病至就诊时间、是否合并慢性病、心功能分级、是否接受溶栓治疗。

Objective To analyze the influencing factors of cardiogenic shock in patients with acute myocardial infarction.Methods A total of 110 patients with acute myocardial infarction admitted to Nanyang First People’s Hospital from March 2021 to May 2022 were selected,and they were divided into control group(55 cases,not complicated by cardiogenic shock)and observation group(55 cases,complicated by cardiogenic shock)according to the occurrence of cardiogenic shock.The clinical data were collected,including gender,age,time from onset to treatment,complicated by chronic diseases,cardiac function classification,thrombolytic treatment,smoking history,and drinking history.The clinical data of the two groups were compared,moreover,items with statistically significant differences were included in the logistic regression equation for calculation,and the influencing factors of cardiogenic shock in patients with acute myocardial infarction were analyzed.Results There was no statistically significant difference in gender,smoking history,and drinking history between the two groups(P>0.05).There were statistically significant differences in age,time from onset to treatment,chronic diseases,cardiac function classification,and thrombolytic treatment between the two groups(P<0.05).The results of the logistic regression equation showed that age,time from onset to treatment,chronic diseases,cardiac function classification,and thrombolytic treatment were influencing factors of cardiogenic shock in patients with acute myocardial infarction(P<0.05).Conclusions The influencing factors of complicated by in patients with acute myocardial infarction mainly include age,time from onset to treatment,chronic diseases,cardiac function classification,and thrombolytic treatment.
