经颅聚焦超声刺激联合认知训练在重症颅脑损伤患者康复中的应用效果

Effect of transcranial focused ultrasound stimulation combined with cognitive training on rehabilitation of patients with severe head injury

摘要 目的观察经颅聚焦超声刺激联合认知训练在重症颅脑损伤患者康复中的应用效果。方法采用前瞻性随机对照研究。抽取2021年1月至2022年12月于郑州大学第一附属医院接受康复治疗的80例重症颅脑损伤患者作为研究对象,采用随机数字表法将其分为对照组与观察组,每组40例。对照组采用认知训练,观察组在对照组基础上加用经颅聚焦超声刺激。比较两组患者治疗前后的血清学指标[脑源性神经营养因子(BDNF)水平]、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分,改良Rankin评分量表(mRS)评分]、认知功能[蒙特利尔认知评估量表(MoCA)评分]和生活自理能力[日常生活能力量表(ADL)评分]。结果治疗后,两组BDNF水平、MoCA评分、ADL评分均高于治疗前,且观察组上述指标高于对照组(P<0.05);治疗后,两组NIHSS、mRS评分均低于治疗前,且观察组低于对照组(P<0.05)。结论重症颅脑损伤患者采用经颅聚焦超声刺激联合认知训练可以有效改善神经功能和认知功能,提高生活自理能力。 Objective To observe the effect of transcranial focused ultrasound stimulation combined with cognitive training on the rehabilitation of patients with severe head injury.Methods This study was s prospective cohort trail.A total of 80 patients with severe head injury who received rehabilitation treatment in the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2022 were selected as the research objects.They were divided into control group and observation group by random number table method,with 40 cases in each group.The control group received cognitive training,while the observation group received transcranial focused ultrasound stimulation based on the intervention of the control group.The serum indicator brain-derived neurotrophic factor(BDNF),neurological function assessed by National Institute of Health stroke scale(NIHSS)and modified Rankin scale(mRS),cognitive function assessed by Montreal cognitive assessment(MoCA)score,and self-care ability assessed by activity of daily living scale(ADL)score were compared between the two groups before and after treatment.Results After treatment,the BDNF level,MoCA score,and ADL score in the two groups were higher than those before treatment,and the above indicators in the observation group were higher than those in the control group(P<0.05).After treatment,the NIHSS score and mRS score of the two groups were lower than those before treatment,and the observation group had lower scores of NIHSS and mRS than the control group(P<0.05).Conclusions The combination of transcranial focused ultrasound stimulation and cognitive training can effectively improve neurological and cognitive function,and enhance self-care ability in patients with severe head injury.
