超声引导下细针穿刺在甲状腺结节患者诊断中的应用价值评估

Evaluation of the application value of ultrasound guided fine needle puncture in the diagnosis of thyroid nodules in patients

目的:探讨超声引导下细针穿刺在甲状腺结节患者诊断中的应用价值。方法:选取医院收治的甲状腺结节患者70例(138个结节)为研究对象,均进行超声引导下细针穿刺检查,以术后病理结果为金标准,观察分析超声引导下细针穿刺检查诊断效能、灵敏度和特异度。138个结节穿刺成功率为86.23%,其中C1占13.77%、C2占5.07%、C3占8.70%、C4占37.68%、C5占34.78%超声引导下细针穿刺检出良性结节36个、恶性结节102个,其中假阴性17个,假阳性2个;病理结果检出良性结节21个、恶性结节117个;超声引导下细针穿刺准确度为86.23%,敏感度85.47%,特异度90.48%,阳性预测98.04%,阴性预测52.78%,与病理诊断一致性(Kappa值)为0.802。结论:超声引导下细针穿刺诊断甲状腺恶性结节与病理结果一致性好,诊断水平较高。

Objective:To explore the application value of ultrasound guided fine needle aspiration in the diagnosis of thyroid nodules in patients.Methods:70 patients(138 nodules)with thyroid nodules admitted to the People's Hospital of Shangyou County in Jiangxi Province were selected as the study subjects.All patients underwent ultrasound-guided fine-needle aspiration examination.The diagnostic efficacy,Sensitivity and specificity of ultrasound-guided fine-needle aspiration examination were observed and analyzed based on the postoperative pathological results as the gold standard.The success rate of puncture for 138 nodules was 86.23%.Ultrasound guided fine needle aspiration detected 36 cases of benign and 102 cases of malignant lesions,of which 17 were false negative and 2 were false positive;Pathological results showed that 21 cases were benign and 117 cases were malignant.The ROC analysis showed that ultrasound-guided fine needle aspiration had a higher value in the diagnosis of thyroid nodules,an accuracy of 86.23%,a sensitivity of 85.47%,a specificity of 90.48%,a positive prediction of 98.04%,a negative prediction of 52.78%,and a consistency with pathological diagnosis(Kappa value)of 0.802.Conclusion:Ultrasound guided fine needle aspiration for the diagnosis of thyroid malignant nodules has good consistency with pathological results,and the diagnostic level is relatively high.
