血清PCSK9、FGF-23表达与特发性矮小症发生的相关性研究

摘要 目的探讨血清前蛋白转化酶枯草溶菌素9(PCSK9)、成纤维细胞生长因子-23(FGF-23)表达水平与特发性矮小症的关系及二者的诊断价值。方法选取2020年8月至2022年10月期间于河南省中医院治疗的80例特发性矮小症患儿作为试验组,同期选取该院84例于儿科就诊的儿童作为对照组,记录儿童体格发育指标。采用酶联免疫吸附试验(ELISA)检测儿童血清PCSK9、FGF-23水平;采用Pearson相关分析检验特发性矮小症患儿血清PCSK9、FGF-23表达水平与体格发育指标的相关性;采用ROC曲线分析血清PCSK9、FGF-23表达水平在特发性矮小症患儿中的诊断价值;采用多因素logistic回归分析影响特发性矮小症发生的因素。结果试验组身高、体重、体重指数(BMI)、瘦素(leptin)、骨钙素(ost)及血清PCSK9、FGF-23表达水平显著低于对照组,差异有统计学意义(P<0.001)。特发性矮小症患儿血清PCSK9与FGF-23水平呈正相关(r=0.474,P<0.05),且二者与体格发育指标中的身高、体重、BMI、leptin、ost均呈正相关(P<0.001)。PCSK9、FGF-23是影响特发性矮小症发生的保护因素(P<0.05)。血清PCSK9、FGF-23单独检测及二者联合诊断特发性矮小症的AUC分别为0.770、0.848、0.891。结论特发性矮小症患儿血清PCSK9、FGF-23表达水平均较低,对特发性矮小症的临床诊断有一定价值。 Objective To investigate relationship between the expression levels of serum preprotein convertase subtilisin/kexin type 9(PCSK9),fibroblast growth factor-23(FGF-23)and idiopathic short stature and their diagnostic value.Methods 80 children with idiopathic short stature who were treated at Henan Provincial Hospital of Traditional Chinese Medicine from August 2020 to October 2022 were selected as experimental group,and 84 children treated in the pediatric department of the hospital during the same period were selected as control group.Physical development indicators of the children were recorded.Enzyme linked immunosorbent assay(ELISA)was used to detect the serum levels of PCSK9 and FGF-23 in children;Pearson correlation analysis was used to test correlation between the expression levels of serum PCSK9 and FGF-23 and physical development indicators in children with idiopathic short stature.ROC curve analysis was used to analyze the diagnostic value of the expression levels of serum PCSK9 and FGF-23 in children with idiopathic short stature;multivariate logistic regression analysis was used to analyze factors influencing the occurrence of idiopathic short stature.Results Height,weight,body mass index(BMI),leptin,osteocalcin(ost)and the expression levels of serum PCSK9 and FGF-23 in the experimental group were obviously lower than those in the control group,and difference was statistically significant(P<0.001).The serum levels of PCSK9 and FGF-23 in children with idiopathic short stature were positively correlated(r=0.474,P<0.05),and both were positively correlated with physical development indicators such as height,weight,BMI,leptin,and ost(P<0.001).PCSK9 and FGF-23 were the protective factors affecting the occurrence of idiopathic short stature(P<0.05).The AUC of serum PCSK9 and FGF-23 detected separately and their combined diagnosis for idiopathic short stature was 0.770,0.848 and 0.891,respectively.Conclusion The expression levels of serum PCSK9 and FGF-23 in children with idiopathic short stature are low,which has certain value for the clinical diagnosis of idiopathic short stature.
