11~13^(+6)孕周胎盘超声监测指标对早发型先兆子痫的预测价值研究

Study on the Predictive Value of Placental Ultrasound Monitoring Indicators in 11-13^(+6)Gestational Weeks for Early-onset Preeclampsia

目的探讨11~13^(+6)孕周胎盘超声监测指标对早发型先兆子痫的预测价值。方法前瞻性分析2022年5—12月于汕头大学医学院接受超声诊断的78例孕妇的临床资料。根据患者后期是否发生先兆子痫分为子痫前期组(n=7)及正常对照组(n=71)。对比两组临床资料,分析胎盘超声监测指标对早发型先兆子痫的预测价值。结果胎盘体积、胎盘血管化指数(vascularization index,Ⅵ)及胎盘血管化血流(vascularized blood flow,VFI)3项联合对早发型先兆子痫的预测价值较高,ROC曲线(receiver operating characteristic,ROC)中AUC为0.901,灵敏度为95.24%、特异度为92.34%。结论11~13^(+6)孕周胎盘超声监测指标胎盘体积、胎盘Ⅵ及VFI对早发型先兆子痫的预测价值较高。

Objective To explore the predictive value of placental ultrasound monitoring indicators for early-onset preeclampsia at 11-13^(+6)gestational weeks.Methods The clinical data of 78 pregnant women who underwent ultrasound diagnosis at Shantou University School of Medicine from May to December 2022 were prospectively analyzed.According to whether the patients developed preeclampsia later in life,they were divided into the preeclampsia group(n=7)and the normal control group(n=71).The clinical data of the two groups were compared,and the predictive value of placental ultrasound monitoring indicators for early-onset preeclampsia was analyzed.Results The combination of placental volume,placental vascularization index(VI)and placental vascularized blood flow(VFI)had higher predictive value for early-onset preeclampsia,with an AUC of 0.901,a sensitivity of 95.24%,and a specificity of 92.34%in the receiver operating characteristic curve.Conclusion Placental ultrasound monitoring indicators of placental volume,placental VI and VFI at 11-13^(+6)weeks of gestation have high predictive value for early-onset preeclampsia.
