中药加味玉液汤治疗气阴两虚型糖尿病肾病慢性肾衰竭疗效及对患者微炎症状态的影响

Efficacy of Chinese Medicine Jiawei Yuye Decoction in Treating Chronic Renal Failure in Diabetic Nephropathy with Qi and Yin Deficiency and Its Effect on Patients'Microinflammatory State

目的探讨中药加味玉液汤在气阴两虚型糖尿病肾病慢性肾衰竭中的疗效以及对患者微炎症状态的作用。方法选取2020年2月—2023年5月阜宁县人民医院收治的85例气阴两虚型糖尿病肾病慢性肾衰竭患者作为研究对象,根据随机数表法分成两组。其中对照组42例采取常规西医治疗,观察组43例采取常规治疗+中药加味玉液汤治疗。两组均连续治疗1个月,对比两组临床疗效。结果观察组治疗有效率为95.35%,高于对照组的80.95%,差异有统计学意义(χ^(2)=4.242,P=0.039)。治疗前,两组肾功能以及炎症因子水平比较,差异无统计学意义(P>0.05);治疗后,观察组血肌酐水平、24 h尿蛋白水平均低于对照组,肾小球滤过率高于对照组,差异有统计学意义(P<0.05)。观察组炎症因子水平低于对照组,差异有统计学意义(P<0.05)。结论中药加味玉液汤的应用,在气阴两虚型糖尿病肾病慢性肾衰竭患者中疗效更为确切,不仅可以有效改善患者肾功能,还能减轻患者炎症反应。

Objective To explore the therapeutic effect of Chinese Traditional Medicine Jiawei Yuye Decoction in chronic renal failure of diabetic nephropathy with Qi and Yin deficiency and its effect on patients'microinflammatory state.Methods A total of 85 patients with Qi-Yin deficiency type diabetic nephropathy and chronic renal failure treated in Funing County People's Hospital from February 2020 to May 2023 were selected as the study objects and divided into two groups according to random number table method.Among them,42 cases in the control group were treated with conventional western medicine,and 43 cases in the observation group were treated with conventional treatment+Chinese Medicine Jiawei Yuye Decoction.Both groups were treated continuously for 1 month,and the clinical efficacy of the two groups was compared.Results The effective rate of the observation group was 95.35%,higher than that of the control group(80.95%),and the difference was statistically significant(χ^(2)=4.242,P=0.039).There was no statistically significant difference in renal function and inflammatory factors between the two groups before treatment(P>0.05);after treatment,the serum creatinine level and 24 h urinary protein level in observation group were lower than those in control group,and the glomerular filtration rate was higher than that in control group,the difference was statistically significant(P<0.05).The level of inflammatory factors in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of Chinese Medicine Jiawei Yuye Decoction has more precise efficacy in patients with chronic renal failure of diabetic nephropathy of Qi and Yin deficiency type,which can not only effectively improve the renal function of patients,but also reduce the inflammatory response of patients.
