磁共振成像在子宫内膜癌术前分期中的诊断价值探析 被引量:1

摘要 目的 分析磁共振成像在诊断子宫内膜癌术前分期中的价值。方法 选取2020年1—12月延边大学附属医院收治的90例疑似子宫内膜癌的患者为研究对象,结合其最终的病理诊断结果作为金标准,对其应用磁共振成像检查的情况进行分析,总结磁共振成像在子宫内膜癌术前分期中的应用价值。结果 90例疑似患者经手术病理诊断,80例确诊为子宫内膜癌患者,磁共振成像确诊真阳76例患者,诊断准确度为95.56%(86/90)、敏感度为95.00%(76/80),特异度为100.00%(10/10)。磁共振成像和手术病理比较计算Kappa为0.845,两者一致性较好。手术病理结果,术后分期为Ⅰa期、Ⅰb期、Ⅱ期、Ⅲ期、Ⅳ期分别有49、21、7、2、1例,应用磁共振成像诊断后,Ⅰa期的诊断符合率为97.96%(48/49),Ⅰb期的诊断符合率为90.48%(19/21),Ⅱ期的诊断符合率为85.71%(6/7),Ⅲ期的诊断符合率为100.00%(1/1),Ⅳ期的诊断符合率为50.00%(1/2)。结论 应用磁共振成像在子宫内膜癌术前分析中准确率高,对子宫肌层浸润程度判断较好,可用于评估患者盆腔及腹腔淋巴结转移情况。 Objective To analyze the value of magnetic resonance imaging in the diagnosis of endometrial carcinoma by preoperative staging.Methods A total of 90 patients with suspected endometrial cancer enrolled in Yanbian University Affiliated Hospital from January to December 2020 were selected as the study objects.Combined with the final pathological diagnosis results as the gold standard,the application of magnetic resonance imaging was compared and analyzed.The value of magnetic resonance imaging in the preoperative staging of endometrial carcinoma were summarized.Results Of the 90 suspected patients,80 were confirmed as endometrial cancer by surgical pathology,and 76 patients were confirmed as Zhenyang by magnetic resonance imaging.The diagnostic accuracy was 95.56%(86/90),sensitivity was 95.00%(76/80),and specificity was 100.00%(10/10).The Kappa of MRI and surgical pathology was 0.845,which showed good consistency.The pathological results showed that there were 49 cases in stageⅠa,21 cases in stageⅠb,7 cases in stageⅡ,2 case in stageⅢand 1 cases in stageⅣ.After the application of magnetic resonance imaging diagnosis,the diagnostic accuracy rate of stageⅠa was 97.96%(48/49),stageⅠb was 90.48%(19/21),stageⅡwas 85.71%(6/7),stageⅢwas 100.00%(1/1),and stageⅣwas 50.00%(1/2).Conclusion Magnetic resonance imaging has high accuracy in the preoperative analysis of endometrial cancer,and can judge the degree of myometrium infiltration well,and can be used to evaluate the status of pelvic and abdominal lymph node metastasis in patients.
