单操作孔胸腔镜手术治疗非小细胞肺癌患者的临床效果及对并发症的影响

Clinical Effect and Complication of Thoracoscopic Surgery with Single Operating Hole in Patients with Non-small Cell Lung Cancer

目的 分析在非小细胞肺癌患者治疗中应用单操作孔胸腔镜手术治疗的临床效果及并发症发生情况。方法 回顾性分析2021年7月—2023年5月在临沂市肿瘤医院胸外科接受手术治疗的82例非小细胞肺癌患者临床资料,依据手术类型不同分为对照组和观察组,各41例。对照组实施单孔胸腔镜手术治疗,观察组实施单操作孔胸腔镜手术,比较两组围手术期指标和并发症发生情况。结果 相较于对照组,观察组手术时长[(187.65±26.74)min]和术后引流管留置时间[(3.27±0.96)d]更短,而术中出血量[(205.87±19.67)mL]和术后引流量[(587.16±72.45)mL]更少,差异有统计学意义(t=0.023、0.009、0.008、0.016,P<0.05)。两组术后12 h的疼痛程度比较,差异无统计学意义(P>0.05)。观察组中转开胸占比低于对照组,但组间差异无统计学意义(P>0.05)。两组术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论 单操作孔胸腔镜手术治疗非小细胞肺癌具有手术效率高和微创的优势,可降低中转开胸风险以及术后引流量,缩短术后引流管留置时间,减少术后并发症。

Objective To analyze the clinical effects and complications of thoracoscopic surgery with single operating hole in the treatment of non-small cell lung cancer.Methods The clinical data of 82 patients with non-small cell lung cancer who underwent surgical treatment in the Department of Thoracic Surgery,Linyi Cancer Hospital from July 2021 to May 2023 were retrospectively analyzed.According to the different types of surgery,they were divided into the control group and the observation group,with 41 cases in each group.The control group was treated with single-port thoracoscopic surgery,and the observation group was treated with single-port thoracoscopic surgery.The perioperative indicators and adverse reactions were compared between the two groups.Results Compared with the control group,the operation time[(187.65±26.74)min]and postoperative drainage tube indwelling time[(3.27±0.96)d]in the observation group were shorter,while the intraoperative blood loss[(205.87±19.67)mL]and postoperative drainage volume[(587.16±72.45)mL]were less,the difference was statistically significant(t=0.023,0.009,0.008,0.016,P<0.05).There was no significant difference in the degree of pain between the two groups 12 hours after operation(P>0.05).The proportion of conversion to thoracotomy in the observation group was lower than that in the control group,but there was no statistically significant difference between the two groups(P>0.05).There was no significant difference in the total incidence of postoperative adverse reactions between the two groups(P>0.05).Conclusion Thoracoscopic surgery with single operating hole has the advantages of high surgical efficiency and minimally invasive,which can reduce the risk of conversion thoracotomy and postoperative drainage flow,shorten the postoperative retention time of drainage tube,and reduce postoperative complications.
