针头放置器在护士胰岛素笔注射规范处理针头中的应用

摘要 目的 设计胰岛素注射笔用针头放置器,并评价其在护士规范处理胰岛素注射笔用针头操作中的应用效果。方法 选择2017年4月—2021年5月参加本院糖尿病专科护理小组组织的胰岛素注射笔操作考核的456名护士作为研究对象,按参加考核时间分为对照组(2017年4月—2019年4月)和实验组(2019年5月—2021年5月)两组,每组各228名。两组均接受过胰岛素注射笔操作的培训,实验组所在科室配备胰岛素注射笔用针头放置器,并接受过针头放置器使用的培训。比较两组护士单手回套外针帽处理使用后的胰岛素注射笔用针头的达标率、处理使用后的胰岛素注射笔用针头的耗时,评价科室放置器规范管理达标率、护士使用放置器处理胰岛素注射笔用针头依从率及满意率。结果 实验组护士达标率高于对照组,差异具有统计学意义(P<0.05);实验组护士处理胰岛素注射针头耗时短于对照组,差异具有统计学意义(P<0.05);实验组护士科室放置器管理平均达标率为96.49%;护士使用放置器处理胰岛素注射笔用针头依从率为94.59%;护士使用放置器处理胰岛素注射针头总体平均满意率为97.14%。结论 胰岛素注射笔用针头放置器制作简单,应用该装置可提高护士处理胰岛素注射笔用针头依从率及满意率,促进护士规范处理使用后的胰岛素注射笔用针头,从而减少针刺伤的发生。 Objective To design a needle placement device for insulin injection pen and evaluate its application efficacy in nurses'standard operation of insulin injection pen needle.Methods A total of 458 nurses who participated in the insulin injection operation assessment that organized by the diabetes specialist nursing group of our hospital from April 2017 to May 2021 were selected.According to the time they participated in the assessment,the nurses were divided into control group(from April 2017 to April 2019)and experimental group(from May 2019 to May 2021),with 228 nurses in each group.Both groups of nurses have received training for insulin injection pen operation,and the nurses of the experimental group were equipped with insulin injection pen needle placement device and received training on the use of it.The compliance rate and time consuming of the nurses for one-handed returning used insulin injection pen needles were observed and compared between the two groups.The compliance rate of standardized management of the needle placement device in the departments,and the compliance rate and satisfaction rate of nurses use the needle placement device to handle insulin injection pen needles were compared.Results The compliance rate of nurses in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05).The time the nurses needed for the dispose of insulin injection pen needles in the experimental group was shorter than the control group,the difference was statistically significant(P<0.05).The average standard management rate in the departments of experimental was 96.49%,the compliance rate of nurses using placement device to dispose insulin injection pens was 94.59%,and the overall average satisfaction rate was 97.14%.Conclusions The needle placement device of insulin injection pen is easy to make.The application of the device can improve the compliance rate and satisfaction rate of nurses in handling insulin injection pen needles,and promote nurses to standardize the use of insulin injection pen needles,thereby reducing the occurrence of needle stick injuries.
