摘要
为扩大消融范围,有效降低组织碳化或气化风险,本研究设计了一种冷却射频消融仪及其控制算法,可实现一次消融下,消融范围直径大于3 cm且接近于标准球形。基于离体牛肝,本研究对比了传统恒功率射频消融、冷却恒功率射频消融和算法控制冷却射频消融三种模式。结果显示,本研究算法控制下,冷却射频消融的椭圆指数与规则指数的最大偏差值最小,椭圆度指数和规则性指数的最大偏差分别为8%、42%,椭圆指数与规则指数的平均值接近于1,分别为1.03和0.72;此外,椭圆指数与规则指数的方差最小,分别为0.0023和0.0012。本研究自制的冷却射频消融仪可有效降低电极接触组织温度,其控制算法与传统和冷却恒功率消融相比更稳定,更趋于球形,且规则程度更高。该研究有望为后续的消融治疗产生积极影响。
In order to extend the ablation range and effectively reduce the risk of tissue carbonization or gasification,we designed a cooling radiofrequency ablation instrument and control algorithm that can achieve ablation ranges greater than 3 cm in diameter and close to standard sphericity with only one ablation.Based on the in vitro beef liver,we compared the traditional constant-power radiofrequency ablation,cooling constant-power radiofrequency ablation,and algorithmically controlled cooling radiofrequency ablation.The results showed that the maximum deviation between the ellipticity index and the regularity index of radiofrequency ablation under control algorithm was the smallest,the maximum deviation of the ellipticity index and regularity index was 8%and 42%,respectively.The average value of the elliptical index and the regular index was the closest to 1,which was 1.03 and 0.72,respectively.In addition,the variance of the elliptical index and the regular index was the smallest,which was 0.0023 and 0.0012,respectively.The self-made cooling radiofrequency ablation instrument could effectively reduce the electrode contact tissue temperature,and its control algorithm was more stable,spherical,and regular than that of traditional and cooling constant power ablation.This research is expected to have a positive impact on subsequent ablation therapy.
