摘要
目的:探讨盆底三维超声成像技术在盆腔脏器脱垂诊断中的应用效果。方法:选取2020年12月—2022年12月南通大学附属医院如皋分院行超声检查且有完整的三维超声成像报告的患者510例为研究对象,其中确诊为盆腔器官脱垂患者290例归为研究组,确诊普通妇科疾病且排除盆腔肿瘤疾病的患者220例归为对照组。比较两组静息状态下、Valsalva动作下及肛提肌收缩状态下的矢状面肛提肌裂孔长度及肛提肌裂孔面积,分析盆底三维超声成像技术的诊断效能。结果:研究组在静息状态下、Valsalva动作下及肛提肌收缩状态下肛提肌裂孔长度和面积均高于对照组,差异有统计学意义(P<0.001);盆底三维超声诊断盆腔器官脱垂准确度、灵敏度、特异度分别为95.86%、95.83%、95.89%。结论:盆底三维超声可清晰显示女性盆腔器官脱垂患者盆底解剖结构,通过观察肛提肌裂孔长度和面积帮助诊断,其诊断准确度、灵敏度、特异度较高,是一种有效的影像学手段。
Objective:To explore the application effect of three dimensional ultrasound imaging of pelvic floor in the diagnosis of pelvic organ prolapse.Methods:A total of five hundred and ten patients with complete 3D ultrasound imaging reports who under⁃went ultrasound examination in Rugao Branch of Affiliated Hospital of Nantong University from December 2020 to December 2022 were selected as the study objects.Among them,two hundred and ninety patients diagnosed with pelvic organ prolapse were classified as the study group,and two hundred and twenty patients diagnosed with common gynecological diseases and excluded pelvic tumor diseases were classified as the control group.The length and area of hiatus of levator anal muscle on sagittal plane were compared between the two groups in resting state,Valsalva movement and contraction of levator anal muscle.The diagnostic efficiency of three dimensional ultrasonic imaging of pelvic floor were analyzed.Results:The length and area of the hiatus of the levator anal muscle in the study group were higher than those in the control group under the resting state,the Valsalva movement and the contraction of the levator anal muscle,and the difference was statistically significant(P<0.001);The accuracy,sensitivity and specificity of three-dimensional pelvic floor ultrasound in the diagnosis of pelvic organ prolapse were 95.86%,95.83%and 95.89%,respectively.Conclusion:Three dimensional ultrasound of pelvic floor can clearly display the anatomical structure of pel⁃vic floor in female patients with pelvic organ prolapse,and help diagnosis by observing the length and area of the levator anal hia⁃tus.The diagnostic accuracy,sensitivity and specificity are high,and it is an effective imaging means.
