基于综合消肿治疗的协同护理模式在乳腺癌术后上肢淋巴水肿患者居家管理中的应用

Application of Collaborative Nursing Model Based on Comprehensive Decongestive Therapy in the Home Management of Patients with Upper Extremity Lymphedema after Breast Cancer Surgery

目的:探讨基于综合消肿治疗(CDT)的协同护理模式在乳腺癌术后上肢淋巴水肿患者居家管理中的应用效果。方法:选取2021年1月—2022年12月南京医科大学附属江苏盛泽医院收治的女性乳腺癌改良根治术后上肢淋巴水肿患者76例为研究对象,根据随机数字表法分为对照组和试验组,各38例。两组均行CDT治疗,对照组行常规护理,试验组行协同护理模式。比较两组患肢周径比率、依从性及满意度。结果:出院后12、24周,试验组患肢周径比率低于对照组,差异有统计学意义(P<0.05);出院后4、12、24周,试验组治疗依从性高于对照组,差异有统计学意义(P<0.05);出院后12、24周,试验组满意度高于对照组,差异有统计学意义(P<0.05)。结论:基于CDT的协同护理模式能有效改善乳腺癌术后淋巴水肿,提高患者治疗依从性及满意度。

Objective:To investigate the application effect of collaborative nursing model based on comprehensive decongestive therapy(CDT)in the home management of patients with upper extremity lymphedema after breast cancer surgery.Methods:Seventy-six female breast cancer patients with upper extremity lymphedema after modified radical mastectomy admitted to Jiangsu Shengze Hospital Affiliated to Nanjing Medical University from January 2021 to December 2022 were selected for the study,and were divided into a control group and an experimental group of thirty-eight cases each according to the randomized numerical table method.Both groups were treated with CDT,the control group was treated with conventional nursing,and the experimental group was treated with collaborative nursing model.The ratio of circumference of affected limbs,compliance and satisfaction were compared between the two groups.Results:12 and 24 weeks after discharge,the ratio of circumference of affected limbs in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05);4,12 and 24 weeks after discharge,the treatment adherence of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05);12 and 24 weeks after discharge,the satisfaction of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Collaborative nursing model based on CDT can effectively improve postoperative lymphedema after breast cancer,and improve patients'treatment adherence and satisfaction.
