血清血管内皮生长因子、25羟维生素D_(3)联合肿瘤标志物诊断子宫内膜癌的临床价值

目的探讨血清中血管内皮生长因子(VEGF)、25羟维生素D_(3)[25(OH)D_(3)]联合肿瘤标志物对子宫内膜癌的诊断价值。方法选取2020年1月一2022年6月该院收治的子宫内膜癌患者、子宫良性病变患者各62例,选择62例同期人院体检的健康女性作为对照组,比较上述3组以及不同病理分级、不同FICO分期子宫内膜癌患者血清VECF、25(OH)D_(3)、HE4、CA19-9、CA125表达水平。结果果子宫内膜癌组、良性病变组、对照组血清VEGF、HE4、CA19-9、CA125水平依次降低(P<0.05),25(OH)D_(3)水平依次升高(P<0.05)。病理分级为G3级、G2级、G1级的患者血清VECF、HE4、CA19-9、CA125水平依次降低(P<0.05),血清25(OH)D_(3)水平依次升高(P<0.05)。子宫内膜癌FIGO分期为IV期、Ⅲ期、IⅡI期、I期患者血清VECF、HE4、CA19-9、CA125水平依次降低(P<0.05),血清25(OH)D_(3)水平依次升高(P<0.05)。ROC分析结果显示,血清VEGF、25(OH)D_(3)、HE4、CA19-9以及CA125联合诊断子宫内膜癌的AUC为0.882,灵敏度为82.3%,特异度为80.6%。结论血清VEGF、25(OH)D_(3)、HE4、CA19-9、CA125表达水平与子宫内膜癌的发生、发展密切相关,可将其联合用于子宫内膜癌的辅助诊断。

Objective This paper aims to investigate the diagnostic value of serum vascular endothelial growth factor(VECF),25 hydroxyvitamin D_(3)[25(OH)D_(3)]and tumor markers in endometrial carcinoma.Methods Sixty-two patients with endom-etrial cancer and 62 patients with benign uterine lesions were selected from Longyou County People's Hospital from January 2020 to June 2022,and 62 healthy women who were admitted to the hospital for physical examination at the same time were selected as controls.The levels of serum VECF,25(OH)D_(3),HE4,CA19-9,CA125 in the above three groups and patients with endo-metrial cancer of different pathological grades and FIGO stages were compared.Results The levels of serum VEGF,HE4,CA19-9 and CA125 in endometrial carcinoma group,benign lesion group and the control group decreased in turn(P<0.05),and the levels of 25(OH)D_(3)increased in turn(P<0.05).The levels of VECF,HE4,CA19-9,CA125 in serum of endome-trial carcinoma patients with pathological grades of C3,G2 and GI decreased in turn(P<0.05),and the levels of 25(OH)D_(3)in serum increased in turn(P<0.05).The serum VECF,HE4,CA19-9 and CA125 levels in patients with endometrial car-cinoma in FICO stages of IV,II,II and I decreased in turn(P<0.05),and the serum 25(OH)D_(3)levels increased in turn(P<0.05).ROC analysis showed that the AUC of VECF,25(OH)D_(3),HE4,CA19-9 and CA125 in the combined diagnosis of endometrial cancer was 0.882,with a sensitivity of 82.3%and a specificity of 80.6%.Conclusion The levels of VEGF,25(OH)D_(3),HE4,CA19-9,CA125 in serum are closely related to the occurrence and development of endometrial carcinoma,which can be used in the auxiliary diagnosis of endometrial carcinoma.
