摘要
目的:分析血清胱抑素C(CysC)、中性粒细胞/淋巴细胞比值(NLR)和血小板/淋巴细胞比值(PLR)与子痫前期严重程度的相关性。方法:收集2020年1月至2022年8月在石河子大学第一附属医院产科分娩并确诊为子痫前期孕妇278例,其中轻度子痫前期组116例,重度子痫前期组162例,以及同期同孕龄健康孕妇173例为健康对照组,比较不同组研究数据和实验室参数,采用Logistic多因素回归分析子痫前期的危险因素,受试者工作特征曲线(ROC曲线)评价Cys C、NLR、PLR等指标对子痫前期的预测价值。结果:(1)在孕前身体质量指数(孕前BMI)、新生儿出生体重、住院天数、剖宫产率、妊娠并发症人数、早产率、CysC、NLR、PLR等三组比较,差异有统计学意义(P<0.05);(2)Spearman相关性研究表明,子痫前期的严重程度与CysC、NLR正相关,而与PLR负相关;(3)Logistic回归提示CysC、NLR的升高,PLR的降低是子痫前期发生的危险因素;(4)辅助诊断子痫前期的联合因子、CysC、NLR、PLR曲线下总面积依次为0.788、0.761、0.735、0.697。结论:CysC、NLR、PLR与子痫前期相关,有助于预测、评估子痫前期的严重程度。
Objective:To analyze the correlation between serum cystatin C(CysC),neutrophil/lymphocyte ratio(NLR)and platelet/lymphocyte ratio(PLR)and the severity of preeclampsia.Methods:From January 2020 to August 2022,278 pregnant women who gave birth in the Obstetrics Department of the First Affiliated Hospital of Shihezi University School of Medicine and were diagnosed with preeclampsia were collected,including 116 cases of mild preeclampsia group,162 cases of severe preeclampsia group,and During the same period,173 healthy pregnant women of the same gestational age served as the healthy control group.The research data and laboratory parameters of different groups were compared.Logistic multivariate regression was used to analyze the risk factors of preeclampsia,and the receiver operating characteristic curve(ROC curve)was used to evaluate CysC and NLR,PLR and other indicators for the predictive value of preeclampsia.Results:(1)There were statistically significant differences in pre-pregnancy body mass index(pre-pregnancy BMI),newborn birth weight,length of hospital stay,cesarean section rate,number of pregnancy complications,premature delivery rate,CysC,NLR,and PLR(<0.05);(2)Spearman correlation study showed that the severity of preeclampsia was positively correlated with CysC and NLR,but negatively correlated with PLR;(3)Logistic regression showed that the increase of CysC and NLR,and the decrease of PLR were Risk factors for preeclampsia;(4)The total area under the curve of combined factors,CysC,NLR,and PLR for auxiliary diagnosis of preeclampsia was 0.788,0.761,0.735,and 0.697.Conclusion:CysC,NLR,and PLR are related to preeclampsia,which are helpful to predict and evaluate the severity of preeclampsia.
