减数正畸联合牙周系统治疗错[牙合]畸形伴中度牙周炎的效果

Effect of reduced orthodontic treatment combined with the periodontal system in the treatment of malocclusion with moderate periodontitis

目的 探讨减数正畸联合牙周系统治疗错[牙合]畸形伴中度牙周炎的效果。方法 选取2021年3月-2023年5月62例于该院进行治疗的错[牙合]畸形伴中度牙周炎的患者作为研究对象,62例患者减数牙共126颗,均给予减数正畸联合牙周系统治疗,选取减数牙邻近牙252颗为观察组,252颗减数牙邻牙的同颌对侧同名牙或牙根相似牙为对照组。观察两组牙齿牙周相关指标、治疗前后减数牙牙槽高度、减数牙侧龈沟液炎症因子水平及治疗前后牙周致病菌水平。结果 两组牙齿牙周相关指标减数牙邻牙牙周探诊深度(PD)及龈沟出血指数(SBI)对比无统计学意义(P> 0.05);治疗前后近减数侧牙牙槽高度对比无统计学意义(P> 0.05),治疗后远减数侧牙槽高度较治疗前高(P<0.05);治疗后龈沟液炎症因子水平肿瘤坏死因子-α (TNF-α)、白细胞介素-6 (IL-6)较治疗前低,白细胞介素-10 (IL-10)较治疗前高(P<0.05);治疗后牙周致病菌牙龈卟啉单胞菌(Pg)、福塞斯坦纳菌(Tf)、齿垢密螺旋体(Td)、中间普氏菌(Pi)均较治疗前少(P<0.05)。结论 减数正畸联合牙周系统治疗错[牙合]畸形伴中度牙周炎可有效改善其牙周情况,提升减数牙邻牙牙槽骨高度,缓解炎症水平,减少牙菌斑。

Objective To investigate the effect of reduced orthodontic treatment combined with periodontal system in the treatment of malocclusion with moderate periodontitis.Methods 62 patients with malocclusion with moderate periodontitis from March 2021to May 2023.126 of 62 patients were treated with combined periodontal system.252 adjacent teeth with reduced teeth were selected as the observation group,and 252 teeth with the opposite side of the adjacent teeth were selected as the control group.The periodontalrelated indicators of the two groups,the alveolar height,the level of inflammatory factors in the gingival sulcus fluid and the level of periodontal pathogenic bacteria before and after treatment were observed.Results The comparison of periodontal exploration depth(PD) and gingival crevicular bleeding index(SBI) between the two groups was not significant(P> 0.05);The comparison of alveolar height without the statistical difference(P> 0.05),The height of the alveolar side after treatment was higher than that before treatment(P <0.05);Tumor necrosis factor-α(TNF 41-α),interleukin-6(IL-6) were lower than those before treatment,Interleukin-10(IL-10)was higher than before treatment(P <0.05);After treatment,the periodontal pathogens Porphyromonas gingivalis(Pg),Forestana(Tf),Treponema tartarsum(Td),and Prevotella intermedius(Pi) were less than before treatment(P <0.05).Conclusion The treatment of reduced orthodontics combined with periodontal system with moderate periodontitis can effectively improve the periodontal condition,improve the height of alveolar bone of reduced adjacent teeth,relieve inflammation level and reduce dental plaque.
