分区低压吸脂联合射频及脂肪填充在中下面部年轻化中的应用

Application of zoned low-pressure liposuction combined with radiofrequency and fat flling in mid-lower face rejuvenation

目的探讨分区低负压吸脂联合低能量双极射频及颗粒脂肪填充在中下面部年轻化中的临床效果及安全性。方法回顾性分析自2019年10月至2023年4月,中国医科大学附属第一医院整形外科收治32例(64侧)鼻唇沟凹陷伴中下面部轮廓不佳伴皮肤松弛患者,接受颊部脂肪堆积区低负压吸脂联合低能量双极射频、鼻唇沟及颊外侧凹陷区颗粒脂肪填充治疗。随访6个月,由医师和患者分别评价术后效果,并统计术后并发症情况。结果32例患者中,平均体质量指数(22.2±1.6)kg/m^(2),每区平均吸出的脂肪量(3.6±2.4)ml,每区平均射频能量(1.6±0.5)kJ。术后随访6个月,患者面部轮廓流畅,皮肤弹性增强,皱纹减轻。非手术医师盲评的鼻唇沟严重程度量表(6-point grading scale,GGS)平均得分从(3.5±0.7)分下降至(1.4±0.8)分(P<0.05),面部松弛程度量表(facial laxity rating scale,FLR)下颌缘区平均得分从(5.3±0.6)分下降至(2.4±0.5)分(P<0.05),患者自评满意率为90.6%。术后出现Ⅰ度皮肤烫伤1例,2例患者术后早期出现口角轻度歪斜,经保守治疗3~12周后症状完全消失,其余患者未发生并发症。结论分区进行低负压吸脂联合低能量双极射频及颗粒脂肪填充能有效地改善面颊部脂肪堆积、鼻唇沟(“法令纹”)凹陷老化及轻中度中下面部松垂,是一种值得推广、安全可靠的治疗方式。

Objective To investigate the clinical effect and safety of zoned low negative pressure liposuction combined with low energy bipolar radiofrequency and granular fat filling in mid-lower facial rejuvenation.MethodsFrom October 2019 to April 2023,32 patients(64 sides)with nasolabial depression with mid-lower facial fat accumulation and skin relaxation were retrospectively analyzed.The patients were followed up for 6 months after receiving bucaal low negative pressure liposuction combined with low energy bipolar radio frequency,nasolabial groove and lateral buccal depression granule fat filling treatment.The postoperative effect was evaluated by doctors and patients respectively,and the postoperative complications were counted.Results Among the 32 patients,the mean body mass index was(22.2±1.6)kg/m^(2),the mean fat extracted per zone was(3.6±2.4)ml,and the mean radiofrequency energy per zone was(1.6±0.5)kJ.After 6 months of follow-up,the patient's facial contour was smooth,skin elasticity was enhanced,and wrinkles were reduced.The average score on the 6-point grading scale(GGS)was decreased from(3.5±0.7)to(1.4±0.8)(P<0.05),the average facial laxity rating scale(FLR)score decreased from(5.3±0.6)points to(2.4±0.5)points(P<0.05),the self-rated satisfaction rate of patients was 90.6%.Postoperative I degree skin scald occurred in 1 case,and oral Angle was slightly skewed in 2 cases in the early stage afer surgery.The symptoms disappeared completely after 3 to 12 weeks of conservative treatment,and no complications occurred in the remaining patients.ConclusionThe low negative pressure liposuction combined with low energy bipolar radiofrequency and particle fat flling can effectively improve the fat accumulation in the cheek,the aging of the"law line"in the nasolabial groove and the loose sag in the mid-lower face,which is a safe and reliable treatment method worthy of promotion.
