抗血管内皮生长因子玻璃体腔注射在糖尿病性视网膜病变激光光凝治疗中的应用效果

摘要 目的 探讨抗血管内皮生长因子(VEGF)玻璃体腔注射在糖尿病性视网膜病变激光光凝治疗中的应用效果。方法 选取2021年6月至2022年6月收治的120例糖尿病性视网膜病变患者为研究对象,按照随机数字表法将其分为对照组与观察组,各60例。两组均给予激光光凝治疗,观察组于术前3~5 d给予康柏西普玻璃体腔注射治疗。比较两组的治疗效果。结果 治疗后,两组的最佳矫正视力(BCVA)均升高,且观察组高于对照组(P<0.05)。治疗后,两组的胎盘生长因子(PIGF)水平均降低,色素上皮衍生因子(PEDF)水平均升高,且观察组优于对照组(P<0.05)。观察组的并发症总发生率为8.33%,显著低于对照组的30.00%,差异具有统计学意义(P<0.05)。结论 抗VEGF玻璃体腔注射在糖尿病性视网膜病变激光光凝术中具有显著辅助治疗效果,可减轻视网膜前膜组织损伤,改善细胞因子水平,降低并发症发生率。 Objective To investigate the application effect of intravitreal injection of anti-vascular endothelial growth factor(VEGF)in laser photocoagulation for diabetic retinopathy.Methods A total of 120 patients with diabetic retinopathy admitted from June 2021 to June 2022 were selected as the research objects and divided into control group and observation group according to the random number table method,with 60 cases in each group.The two groups were treated with laser photocoagulation,and the observation group was given intravitreal injection of conbercept at 3-5 d before surgery.The therapeutic effects of the two groups were compared.Results After treatment,the best corrected visual acuity(BCVA)of the two groups increased,and that of the observation group was higher than the control group(P<0.05).After treatment,the level of placental growth factor(PIGF)in the two groups decreased,the level of pigment epithelium derived factor(PEDF)increased,and those in the observation group were better than the control group(P<0.05).The total incidence of complications in the observation group was 8.33%,which was significantly lower than 30.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Intravitreal injection of anti-VEGF has a significant adjuvant therapeutic effect in laser photocoagulation for diabetic retinopathy,which can reduce the damage of epiretinal membrane tissue,improve the level of cytokines and reduce the incidence of complications.
