Angiojet机械血栓清除治疗急性下肢深静脉血栓疗效分析

Therapeutic effect of Angiojet mechanical thrombus clearance on acute deep venous thrombosis of lower extremities

目的:探讨Angiojet机械血栓清除对急性下肢深静脉血栓(LEDVT)伴髂静脉受压综合征(IVCS)患者肢体肿胀及静脉通畅度的影响。方法:选取2020年7月—2022年8月LEDVT伴IVCS患者101例,按照随机数字表法分为对照组(50例)和观察组(51例)。对照组给予导管接触性溶栓(CDT)治疗,观察组在对照组基础上联合Angiojet机械血栓清除治疗。对比两组患者下肢肿胀程度、静脉通畅度、凝血功能及并发症情况。结果:两组术后大腿、小腿患健侧周径差水平较术前降低,且观察组低于对照组(P<0.05);观察组血栓清除率高于对照组(P<0.05);两组术后静脉通畅度评分水平较术前降低,且观察组低于对照组(P<0.05);两组术后D-二聚体(D-D)水平较术前升高,但观察组低于对照组(P<0.05);纤维蛋白原(FIB)水平较术前降低,且观察组高于对照组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:Angiojet机械血栓清除可调节LEDVT伴IVCS患者下肢肿胀程度,改善凝血功能,调节静脉通畅度,安全性好,术后无复发。

Objective:To investigate the effects of Angiojet mechanical thrombus removal on limb swelling and venous patency in patients with acute deep vein thrombosis of lower extremity(LEDVT)with iliac vein compression syndrome(IVCS).Methods:A total of 101 cases of LEDVT patients with IVCS in our hospital from July 2020 to August 2022 were selected as the study objects,50 cases in the control group were given catheter contact thrombilysis(CDT)treatment,and 51 cases in the observation group were given mechanical thrombosis combined with Angiojet treatment.The swelling degree of lower limbs,venous patency,coagulation function and complications were compared between the two groups.Results:The difference of the circumference of the healthy side of the thigh and lower leg after operation was lower than that before operation,and the observation group was lower than the control group(P<0.05).The thrombus clearance rate of observation group was higher than that of control group(P<0.05).The score level of postoperative venous patency in both groups was lower than that before surgery,and the observation group was lower than the control group(P<0.05).The level of D-dimer(D-D)after operation was higher than that before operation,and the observation group was lower than the control group(P<0.05).The level of fibrinogen(FIB)in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Angiojet mechanical thrombectomy can adjust the degree of lower limb swelling in patients with LEDVT accompanied by IVCS,improve coagulation function and regulate venous patency,with good safety and no postoperative recurrence.
