基于快速康复外科理念下穴位贴敷辨证治疗胃肠道术后腹胀的临床研究

Clinical Study on Acupoint Application Treatment of Abdominal Distension after Gastrointestinal Operation Based on the Concept of Rapid Rehabilitation Surgery

目的 分析快速康复外科理念下穴位贴敷辨证治疗胃肠道术后腹胀的临床效果。方法 选取2019年6月—2022年6月泰安市中医医院收治的120例胃肠道术后腹胀患者的临床资料,根据不同治疗方法分为研究组和对照组,各60例。对照组采用常规干预,研究组在对照组基础上联合基于快速康复外科理念的穴位贴敷辨证治疗。对比两组患者干预胃肠功能改善时间、腹胀程度、胃肠功能评分和舒适度评分,以及不良反应发生情况。结果 研究组的肠鸣音恢复时间、排气时间和排便时间均明显短于对照组,差异有统计学意义(P<0.05);研究组的术后5 d腹胀有效改善率明显高于对照组,差异有统计学意义(P<0.05);研究组的胃肠功能评分明显低于对照组,舒适度评分明显高于对照组,差异有统计学意义(P<0.05);两组术后不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 快速康复外科理念下穴位贴敷辨证治疗胃肠道术后腹胀,可改善患者腹胀程度,促使胃肠功能早日恢复,提高患者术后康复舒适度。

Objective To analyze the clinical effect of acupoint application on postoperative abdominal distension of gastrointestinal tract under the concept of rapid rehabilitation surgery.Methods The clinical data of 120 patients with gastrointestinal postoperative abdominal distension admitted to Tai'an Hospital of Traditional Chinese Medicine from June 2019 to June 2022 were retrospectively analyzed,and they were divided into study group and control group according to different treatment methods,with 60 cases in each group.The control group received routine intervention,and the research group received routine intervention combined with dialectical treatment based on the concept of rapid rehabilitation surgery.The improvement time of gastrointestinal function,degree of abdominal distension,gastrointestinal function score and comfort score,as well as the occurrence of adverse reactions were compared between the two groups.Results The bowel sound recovery time,exhaust time and defecation time in the study group were significantly shorter than those in the control group,and the differences were statistically significant (P<0.05).The effective improvement rate of abdominal distension 5 d after operation in the study group was significantly higher than that in the control group,and the difference was statistically significant (P<0.05).The gastrointestinal function score of the study group was significantly lower than that of the control group,and the comfort score was significantly higher,and the differences were statistically significant (P<0.05).There was no statistically significant difference in the incidence of postoperative adverse reactions between the two groups (P>0.05).Conclusion The treatment of postoperative abdominal distension under the concept of rapid rehabilitation surgery can improve the degree of abdominal distension,promote the early recovery of gastrointestinal function,and improve the comfort of postoperative rehabilitation of patients.
