新活素联合诺欣妥治疗慢性心力衰竭的疗效及对心功能的影响 被引量:1

Effect of Neotoxin combined with Shakubactril on patients with chronic heart failure and its influence on cardiac function

摘要 目的探究重组人脑利钠肽(新活素)联合沙库巴曲缬沙坦钠(诺欣妥)治疗慢性心力衰竭(Chronic heart failure,CHF)患者的疗效及对其心功能影响。方法依随机数字法将我院2021年2月至2023年2月间收治的275例CHF患者分为参照组(138例)、研究组(137例)。参照组给予诺欣妥治疗,研究组给予新活素联合诺欣妥治疗。比较两组临床疗效、治疗前后心功能[左室轴缩短率(FS)、左室射血分数(LVEF)、心排出量(CO)、心脏指数(CI)]、冠脉血流动力学[左前降支时间速度积分(LADTVI)、左前降支舒张期峰流速(LADDPV)、左前降支收缩期峰流速(LADSPV)]、心肌损伤标志物[肌钙蛋白T(cTnT)、热休克蛋白27(HSP27)、细胞间黏附分子-l(ICAM-l)、脑钠肽(BNP)]及氧化应激[丙二醛(MDA)、超氧化物歧化酶(SOD)、一氧化氮(NO)]指标。结果研究组临床总有效率(93.43%)高于参照组(81.88%)(P<0.05);治疗后研究组FS、LVEF、CI、CO高于参照组(P<0.05);治疗后研究组LADTVI、LADDPV、LADSPV高于参照组(P<0.05);治疗后研究组cTnT、HSP27、ICAM-l、BNP低于参照组(P<0.05);治疗后研究组MDA低于参照组,NO、SOD高于参照组(P<0.05)。结论诺欣妥联合新活素治疗CHF疗效确切,能减少氧化应激反应,减轻心肌受损,有效改善心功能及冠脉血流动力学。 Objective To explore the effect of recombinant human brain natriuretic peptide(neopeptide)combined with Shakubactriovalsartan sodium(nohintol)in the treatment of chronic heart failure(CHF)patients and its influence on cardiac function.Methods 275 patients with CHF admitted to our hospital from February 2021 to February 2023 were divided into reference group(138 cases)and study group(137 cases)by random number method.The control group was treated with nohindal,and the study group was treated with neotoxin combined with nohindal.The clinical efficacy,cardiac function[left ventricular axial reduction rate(FS),left ventricular ejection fraction(LVEF),cardiac output(CO),and cardiac index(CI)],coronary hemodynamics[left anterior descending branch time velocity integral(LADTVI),left anterior descending branch diastolic peak velocity(LADDPV),left anterior descending(LAD)branch systolic peak velocity SPV)],myocardial injury markers[troponin T(cTnT),heat shock protein 27(HSP27),intercellular adhesion molecule-l(ICAM-l),brain natriuretic peptide(BNP)]and oxidative stress indicators[malondialdehyde(MDA),superoxide dismutase(SOD),nitric oxide(NO)]were compared between the two groups.Results The total effective rate of the study group(93.43%)was higher than that of the control group(81.88%)(P<0.05).After treatment,FS,LVEF,CI and CO in the study group were higher than those in the control group(P<0.05).After treatment,the LADTVI,LADDPV and LADSPV of the study group were higher than those of the control group(P<0.05);After treatment,cTnT,HSP27,ICAM-l and BNP in the study group were lower than those in the control group(P<0.05).After treatment,MDA of the study group was lower than that of the control group,while NO and SOD of the study group were higher than that of the control group(P<0.05).Conclusion Nohintal combined with neoactin is effective in the treatment of CHF,which can reduce oxidative stress response,alleviate myocardial damage,and effectively improve cardiac function and coronary hemodynamics.
