环保脱蜡液用于坏死性肉芽肿性炎抗酸染色的效果

摘要 目的探讨环保脱蜡液对坏死性肉芽肿性炎石蜡切片的抗酸染色效果。方法回顾性收集病理诊断结果为坏死性肉芽肿性炎的163例组织标本作为研究对象,分别使用二甲苯(二甲苯组)和环保脱蜡液(环保脱蜡液组)作为切片的透明脱蜡液。采用齐-内抗酸染色法检测两组切片的结核分枝杆菌,比较两组切片的抗酸染色效果、结核分枝杆菌阳性率和数量及两组切片的结核分枝杆菌的符合率和诊断吻合系数。结果两组切片抗酸染色均背景清晰,呈灰蓝色;细胞核蓝色,胞浆粉红色;结核分枝杆菌清晰可见,呈鲜红色;肺泡腔内壁见粉染渗出物。环保脱蜡液组切片的结核分枝杆菌阳性率高于二甲苯组,差异有统计学意义(χ^(2)=105.20,P<0.001)。两组切片的结核分枝杆菌符合率为87.12%,结核分枝杆菌诊断吻合度较强且差异有统计学意义(kappa=0.723,P<0.001)。环保脱蜡液组检出的结核分枝杆菌均数高于二甲苯组,差异有统计学意义(t=40.106,P<0.001)。结论与二甲苯相比,使用环保脱蜡液有利于提高坏死性肉芽肿性炎石蜡切片的结核分枝杆菌抗酸染色检出率和数量。 Objective To investigate the acid-fast staining effect of environmental friendly dewaxing solution for paraffin sections of necrotizing granulomatous inflammation.Methods A total of 163 tissue sections of pathologically diagnosed necroti-zing granulomatous inflammation were retrospectively divided into two groups:using either xylene or environmental friendly dewaxing solution as transparent dewaxing solution.The Ziehl-Neelsen method was used to detect tuberculous mycobacteria in both groups.The two groups were compared in the acid-fast staining effect and the positive rate and number of tuberculous mycobacteria.The coincidence rate and coincidence coefficient for tuberculous mycobacteria detection of the two groups were calculated.Results The acid-fast-stained sections in both groups showed a clear gray-blue background;blue nuclei surrounded by pink cytoplasm;clearly visible bright red tuberculosis mycobacteria;and pink-stained exudates inside the wall of the alveoli.The positive rate of tuberculosis mycobacteria in the environmental friendly dewaxing solution group was significantly higher than that of the xylene group(χ^(2)=105.20,P<0.001).The coincidence rate of tuberculosis mycobacteria between the two groups was 87.12%.The coincidence degree of tuberculous mycobacteria detection was relatively high,which was statistically significant(kappa=0.723,P<0.001).The mean number of tuberculosis mycobacteria detected in the environmental friendly dewaxing solution group was significantly higher than that in the xylene group(t=40.106,P<0.001).Conclusion Compared with using xylene,the use of environmental friendly dewaxing solution can help improve the positive rate and number of tuberculous mycobacteria detected with acid-fast staining in paraffin sections of necrotizing granulomatous inflammation.
