养阴清肺汤加减雾化吸入在声带息肉术后患者中的应用研究

Application Study of Aerosol Inhalation with Modified Yangyin Qingfei Decoction in Patients After Surgery for Polyp of Vocal Cord

目的:观察养阴清肺汤加减雾化吸入在声带息肉肺肾阴虚证术后患者中的应用效果。方法:选取90例声带息肉肺肾阴虚证患者,采用简单随机法分为观察组和对照组各45例。2组均行支撑喉镜下二氧化碳(CO_(2))激光声带息肉切除术治疗,术后均给予西药治疗及进行康复训练。在上述治疗基础上,对照组术后给予布地奈德混悬液雾化吸入治疗,观察组给予养阴清肺汤加减雾化吸入治疗,2组均治疗14 d。比较2组临床疗效、中医证候积分、嗓音声学指标及不良反应发生率。结果:治疗后,观察组治愈率77.78%,高于对照组57.78%(P<0.05)。2组声嘶日久、咽喉干燥、口干积分均较治疗前降低(P<0.05),观察组上述3项中医证候积分均低于对照组(P<0.05)。2组基频微扰(Jitter)、振幅微扰(Shimmer)、声门噪声能量(NNE)均较治疗前降低(P<0.05),观察组Jitter、Shimmer、NNE均低于对照组(P<0.05)。治疗期间,观察组不良反应发生率2.22%,与对照组4.44%比较,差异无统计学意义(P>0.05)。结论:养阴清肺汤加减雾化吸入用于声带息肉肺肾阴虚证术后的治疗效果显著,可有效改善患者的临床症状,助其恢复正常发音,安全性较好。

Objective:To observe the application effect of aerosol inhalation with modified Yangyin Qingfei Decoction in patients with lung-kidney yin deficiency syndrome after surgery for polyp of the vocal cord.Methods:A total of 90 cases of patients with polyp of the vocal cord with lung-kidney yin deficiency were selected and divided into the observation group and the control group according to the simple random method,with 45 cases in each group.Both groups were treated with carbon dioxide(CO_(2))laser vocal cord polypectomy under laryngoscope.After surgery,both groups were given western medicine treatment and rehabilitation training.The control group was additionally treated with aerosol inhalation with Budesonide Suspension Liquid,and the observation group was additionally treated with aerosol inhalation with modified Yangyin Qingfei Decoction.Both groups were treated for 14 days.The clinical effects,traditional Chinese medicine(TCM)syndrome scores,vocal parameters,and incidence of adverse reactions were compared between the two groups.Results:After treatment,the cure rate was 77.78%in the observation group,higher than that of 57.78%in the control group(P<0.05).The scores of hoarseness for a long time,dry throat,and dry mouth in the two groups were decreased when compared with those before treatment(P<0.05).The above three TCM syndrome scores in the observation group were lower than those in the control group(P<0.05).The fundamental frequency perturbation(Jitter),amplitude perturbation(Shimmer),and normalized noise energy(NNE)in the two groups were all reduced when compared with those before treatment(P<0.05),and Jitter,Shimmer,and NNE in the observation group were lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions was 2.22%in the observation group,and 4.44%in the control group,there being no significant difference(P>0.05).Conclusion:Aerosol inhalation with modified Yangyin Qingfei Decoction has a significant curative effect in treating patients with lung-kidney yin deficiency syndrome after surgery for polyp of the vocal cord,which can effectively improve the clinical symptoms of patients and help them recover normal vocalization,with good safety.
