芒硝外敷联合生大黄保留灌肠治疗重症急性胰腺炎临床研究

Clinical Study on External Application of Natrii Sulfas Combined with Retention Enema with Rhei Radix et Rhizoma for Severe Acute Pancreatitis

目的:观察芒硝外敷联合生大黄保留灌肠治疗重症急性胰腺炎(SAP)的临床疗效。方法:选取144例SAP患者,按随机数字表法分为常规组和联合组各72例。常规组给予常规治疗,联合组在常规组基础上给予芒硝外敷联合生大黄保留灌肠治疗。2组均治疗7 d。比较2组肠鸣音恢复、腹胀缓解、总住院时间,以及血清淀粉酶(AMS)、脂肪酶(LPS)、白细胞介素-6 (IL-6)、降钙素原(PCT)水平。结果:联合组肠鸣音恢复、腹胀缓解、总住院时间均短于常规组,差异均有统计学意义(P<0.05)。治疗后,2组血清AMS、LPS、IL-6、PCT水平均较治疗前降低,联合组血清AMS、LPS、IL-6、PCT水平均低于常规组,差异均有统计学意义(P<0.05)。结论:芒硝外敷联合生大黄保留灌肠治疗SAP能够缓解患者的临床症状,降低血清AMS、LPS、IL-6、PCT水平。

Objective:To observe the clinical effect of external application of Natrii Sulfas combined with retention enema with Rhei Radix et Rhizoma for severe acute pancreatitis(SAP).Methods:A total of 144 cases of SAP patients were selected and divided into the routine group and the combination group according to the random number table method,with 72 cases in each group.The routine group was given routine treatment,and the combination group was additionally treated with external application of Natrii Sulfas combined with retention enema with Rhei Radix et Rhizoma based on the treatment of the routine group.Both groups were treated for 7 days.The time of bowel sounds recovery,abdominal distension relief and total hospital stay,as well as the levels of amylase(AMS),lipase(LPS),interleukin-6(IL-6)and procalcitonin(PCT)in serum were compared between the two groups.Results:The time of bowel sounds recovery,abdominal distension relief and total hospital stay in the combination group were shorter than those in the routine group,differences being significant(P<0.05).After treatment,the levels of AMS,LPS,IL-6 and PCT in serum in the two groups were decreased when compared with those before treatment,and the levels of AMS,LPS,IL-6 and PCT in serum in the combination group were lower than those in the routine group,differences being significant(P<0.05).Conclusion:External application of Natrii Sulfas combined with retention enema with Rhei Radix et Rhizoma for SAP can relieve clinical symptoms,and reduce levels of AMS,LPS,IL-6 and PCT in serum.
