标本溶血对电解质、心肌酶、肝功及血脂部分项目水平的影响观察

摘要 目的探究标本溶血在生化检验中对电解质、心肌酶、肝功及血脂水平的影响。方法抽取禹州市中医院2022年6月至2023年6月接受生化检验的78例体检者作为本次研究对象,所有受检样本随机分为两份,一份进行溶血处理,对两份样本的生化检验指标进行检测并对比。结果溶血后,体检者的钾、氯与钙的水平较溶血前均升高,两者差异存在统计学意义(P<0.05),而钠水平在溶血前、后未发生明显变化,差异无统计学意义(P>0.05)。体检者溶血后的肌酸激酶、肌酸激酶同工酶与N末端B型利钠肽前体水平较溶血前高,差异具有统计学意义(P<0.05)。溶血后,体检者的丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、总胆红素与总蛋白水平较溶血前高,差异具有统计学意义(P<0.05)。体检者在溶血后的总胆固醇与甘油三酯水平均高于溶血前,差异具有统计学意义(P<0.05),但体检者高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平在溶血前、后均未出现明显区别,差异无统计学意义(P>0.05)。结论标本溶血会造成体检者生化检验中多项指标发生变化,对生化检查结果造成影响,不利于后续对疾病的诊断与治疗。临床检验中应加强生化检验质量控制,尽可能避免标本出现溶血情况,提升生化检验结果的准确性。 Objective To investigate the effects of specimen hemolysis on electrolyte,myocardial enzymes,liver function,and blood lipid levels in biochemical tests.Methods A total of 78 physical examinees who underwent biochemical tests at Yuzhou Traditional Chinese Medicine Hospital from June 2022 to June 2023 were selected as the study subjects.All test samples were randomly divided into two parts,one of which was subjected to hemolysis treatment.The biochemical test indicators of the two samples were tested and compared.Results After hemolysis,the levels of potassium,chlorine,and calcium in the examinees increased compared to those before hemolysis,and the difference between the two was statistically significant(P<0.05),while the sodium level did not show significant changes before and after hemolysis,and the difference was not statistically significant(P>0.05).The levels of creatine kinase,creatine kinase isoenzyme,and N-terminal B-type natriuretic peptide precursor in the examinees after hemolysis were higher than those before hemolysis,and the differences were statistically significant(P<0.05).After hemolysis,the levels of alanine aminotransferase,aspartate aminotransferase,total bilirubin,and total protein in the examinees were higher than those before hemolysis,and the differences were statistically significant(P<0.05).The total cholesterol and triglyceride levels of the examinees after hemolysis were higher than those before hemolysis,and the differences were statistically significant(P<0.05).However,there was no significant difference in the levels of high-density lipoprotein cholesterol and low-density lipoprotein cholesterol between examinees before and after hemolysis,and the differences were not statistically significant(P>0.05).Conclusion Hemolysis of the specimen can cause changes in multiple indicators in the biochemical examination of the examinee,which will affect the results of the biochemical test and be disadvantageous to the subsequent diagnosis and treatment of the disease.In clinical testing,quality control of biochemical tests should be strengthened to avoid hemolysis in specimens as much as possible and improve the accuracy of biochemical test results.
