Epley复位法时增加Hallpike位后倒次数治疗PC-BPPV患者的疗效研究

Study on efficacy of increased times of backward tilt to Hallpike position during Epley maneuver in the treatment of PC-BPPV

目的对比Epley复位法时增加Hallpike位后倒次数与Epley复位法治疗后半规管良性阵发性位置性眩晕(PC-BPPV)患者的疗效。方法130例PC-BPPV患者,随机分为对照组和观察组,每组65例。对照组行Epley复位法复位治疗,观察组采用Epley复位法时增加Hallpike位后倒次数复位治疗。对比两组及时、短期及长期的疗效。结果观察组及时治愈率66.15%明显高于对照组的46.15%,差异有统计学意义(P<0.05)。两组及时总有效率对比差异无统计学意义(P>0.05)。观察组短期治愈率93.85%明显高于对照组的75.38%,差异有统计学意义(P<0.05)。两组短期总有效率对比差异无统计学意义(P>0.05)。两组长期治愈率、总有效率对比,差异无统计学意义(P>0.05)。结论利用眩晕诊疗仪进行PC-BPPV复位时,在Epley复位法时增加Hallpike位后倒次数具有较高的及时、短期治愈率,可在短期内显著改善患者眩晕症状,减轻患者痛苦,减少医疗支出。

Objective To compare the efficacy of increased times of backward tilt to Hallpike position during Epley maneuver in the treatment of posterior semicircular canal benign paroxysmal positional vertigo(PCBPPV).Methods 130 patients with PC-BPPV were randomly divided into a control group and an observation group,with 65 cases in each group.The control group was treated with Epley maneuver,and the observation group was treated with increased times of backward tilt to Hallpike position during Epley maneuver.The immediate,short-term and long-term efficacy of the two groups were compared.Results The immediate cure rate of 66.15%in the observation group was significantly higher than that of 46.15%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the total immediate effective rate between the two groups(P>0.05).The short-term cure rate of 93.85%in the observation group was significantly higher than that of 75.38%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in total short-term effective rate between the two groups(P>0.05).There was no statistically significant difference in long-term cure rate and total effective rate between the two groups(P>0.05).Conclusion When conducting PC-BPPV repositioning with vertigo diagnostic instrument,increasing the times of backward tilt to Hallpike position has a higher rate of immediate and short-term cure rate,which can significantly improve patients'vertigo symptoms in a short run,relieve patients'pain and reduce medical expenses.
