阿戈美拉汀联合坦度螺酮对老年抑郁症患者血清去甲肾上腺素及5-羟色胺与抑郁状态的影响

Effects of agomelatine combined with tandospirone on serum norepinephrine,5-hydroxytryptamine and depressive state in elderly patients with depression

目的探究阿戈美拉汀联合坦度螺酮应用于老年抑郁症患者的效果。方法选取2020年3月至2021年9月青岛市精神卫生中心收治的76例老年抑郁症患者作为研究对象,采用随机数字表法分为对照组与观察组,各38例。对照组给予坦度螺酮治疗,观察组给予阿戈美拉汀联合坦度螺酮治疗。比较两组临床疗效、去甲肾上腺素(NE)、5-羟色胺(5-HT)、抑郁状态[汉密尔顿抑郁量表(Hamilton depression scale,HAMD)评分]、睡眠质量[匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)]、认知功能及安全性。结果观察组治疗总有效率为94.74%,高于对照组的76.32%,差异有统计学意义(P<0.05)。治疗前,两组血清NE、5-HT水平比较差异无统计学意义;治疗后,两组血清NE、5-HT水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。治疗前,两组HAMD、PSQI评分比较差异无统计学意义;治疗后,两组HAMD、PSQI评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组命名、注意、语言、延迟记忆、记忆、抽象、视空间与执行、定向力评分比较差异无统计学意义;治疗后,两组命名、注意、语言、延迟记忆、记忆、抽象、视空间与执行、定向力评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论阿戈美拉汀联合坦度螺酮治疗老年抑郁症效果显著,可提高患者血清NE、5-HT水平,改善其抑郁状态、认知功能及睡眠质量,安全性较高,值得临床推广应用。

Objective To explore the effect of agomelatine combined with tandospirone on elderly patients with depression.Methods 76 elderly patients with depression admitted to the Psychological Hospital of Qingdao Mental Health Center from March 2020 to September 2021 were selected as the research subjects,and they were divided into the control group and the observation group by random number table method,with 38 cases in each group.The control group was given tandospirone treatment,and the observation group was given agomelatine combined with tandospirone treat-ment.The clinical efficacy,norepinephrine(NE),5-hydroxytryptamine(5-HT),depressive state(Hamilton depression scale[HAMD]),sleep quality(Pittsburgh sleep quality index[PSQI]),cognitive function and safety were compared between the two groups.Results The total effective rate in the observation group was 94.74%,which was higher than 76.32%in the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in serum NE and 5-HT levels between the two groups;after treatment,the levels of serum NE and 5-HT of the two groups were higher than before treatment,and the observation group was higher than the control group,the differences were statistical-ly significant(P<0.05).Before treatment,there were no significant differences in HAMD、PSQI scores between the two groups;after treatment,the scores of HAMD、PSQI of the two groups were lower than before treatment,and the observation group was lower than the control group,the differenc-es were statistically significant(P<0.05).Before treatment,there were no significant differences in naming,attention,language,delayed memory,memory,abstraction,visuospatial and executive scores,and orientation scores between the two groups;after treatment,the scores of naming,attention,language,delayed memory,memory,abstraction,visuospatial and executive,and orientation of the two groups were higher than before treatment,and the observation group was higher than the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Agomelatine combined with tandospirone has significant effect in the treatment of senile depression,can increasing the serum NE and 5-HT levels,improving the depressive depression state,cognitive function and sleep quality,it is safe and worthy of clinical promotion and application.
