摘要
目的探究红细胞储存时间对地中海贫血患者红细胞输注疗效的影响。方法选取2020年1月至2021年6月于萍乡市人民医院接受治疗的80例地中海贫血患者作为研究对象,按照随机数字表法分为观察组与对照组,各40例。对照组予以储存时间>14 d红细胞进行红细胞输注,观察组予以<14 d红细胞进行红细胞输注,比较两组输注前后血清血液指标[血红蛋白(Hb)、红细胞计数(RBC)、网织红细胞计数(RET)、血清铁蛋白(SF)]、输血3 d时有效输注率、血清相关因子(K^(+)、Na^(+)、Cl^(-))水平及不良反应发生情况。结果输血1 d后,两组RBC、Hb水平均高于输血前,RET、SF水平均低于输血前,且观察组RBC、Hb水平均高于对照组,RET、SF水平均低于对照组,差异有统计学意义(P<0.05)。输血3 d后,观察组红细胞输注有效率为92.50%,高于对照组的72.50%,差异有统计学意义(P<0.05)。输血1 d后,两组K+、Na+、Cl-水平均高于输血前,但观察组K+水平低于对照组,Na+水平高于对照组,差异有统计学意义(P<0.05),两组Cl-水平比较差异无统计学意义。观察组不良反应发生率为12.50%,低于对照组的32.50%,差异有统计学意义(P<0.05)。结论使用储存时间较短的红细胞对地中海贫血患者进行悬浮红细胞输注效果较好,且对器官损伤较小,具有较高安全性,值得临床推广应用。
Objective To explore the effect of red blood cell storage time on the efficacy of red blood cell transfusion in patients with thalassemia.Methods 80 patients with thalassemia who were treated in Pingxiang People's Hospital from January 2020 to June 2021 were selected as the research subjects,they were divided into the observation group and the control group according to the random number table method,with 40 cases in each group.The control group was given red blood cells with storage time>14 d for red blood cell transfusion,and the observation group was given red blood cells with storage time<14 d for red blood cell transfusion,the serum blood indexes(hemoglobin[Hb],red blood cell[RBC],reticulocyte[RET],serum ferritin[SF]),effective transfusion rate at 3 d after transfusion,serum related factors(K^(+),Na^(+),Cl^(-))levels and adverse reactions were compared between the two groups.Results After 1 d of blood transfusion,the levels of RBC and Hb of the two groups were higher than those before blood transfusion,and the levels of RET and SF were lower than those before blood transfusion,and the levels of RBC and Hb in the observation group were higher than those in the control group,and the levels of RET and SF were lower than those in the control group,the differences were statistically significant(P<0.05).After 3 d of blood transfusion,the effective rate of red blood cell transfusion in the observation group was 92.50%,which was higher than 72.50%in the control group,and the difference was statistically significant(P<0.05).After 1 d of blood transfusion,the levels of K^(+),Na^(+),Cl^(-)of the two groups were higher than those before blood transfusion,but the level of K+in the observation group was lower than that in the control group,and the level of Na+was higher than that in the control group,the differences were statistically significant(P<0.05),there was no significant difference in Cl-level between the two groups.The incidence of adverse reactions in the observation group was 12.50%,which was lower than 32.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion The use of red blood cells with shorter storage time for suspended red blood cell transfusion in patients with thalassemia is better,and has less organ damage and higher safety,and is worthy of clinical promotion and application.
