年轻乳腺癌不同分子亚型的超声多模态特征

摘要 目的分析年轻乳腺癌不同分子亚型的超声多模态特征。方法选取经病理证实的211例年轻乳腺癌患者,依据免疫组织化学结果进行分子分型,应用常规超声、三维(ABVS)、彩色多普勒血流成像及弹性技术,分析总结年轻乳腺癌不同分子分型的超声征象进行比较。结果分析显示不同分子分型乳腺癌间的年龄均数差异无统计学意义(F=1.337,P=0.263)。年轻乳腺癌不同分子分型间在肿块与非肿块型间、形状、生长方式、边缘特征、冠状面特点、后方回声、彩色血流、阻力指数及有无腋窝淋巴结转移间差异均无统计学意义(P>0.05),但在病灶内有无钙化及弹性评分间差异均有统计学意义(P<0.05);不同分子分型的病理类型比较,差异有统计学意义(χ^(2)=14.549,P=0.002)。结论不同分子分型的年轻乳腺癌有不同的超声征象,对于区分其分子分型可以提供一定帮助,有助于乳腺癌患者治疗和预后评估。 Objective To analyze the ultrasound multimodal characteristics of different molecular subtypes of breast cancer in young women.Methods We retrospectively analyzed 211 young women with breast cancer confirmed by pathology.Molecular types were classified by immunohistochemical results.We applied conventional ultrasound,three-dimensional(ABVS),color Doppler blood flow imaging and elasticity techniques,analyzed and summarized different molecular types of ultrasound signs of breast cancer in young women.Results The analysis showed that there was no significant difference in the mean age between different molecular types(F=1.337,P=0.263).Also,there was no statistical difference between the different molecular types of breast cancer in the tumor and non-mass type,shape,growth pattern,edge characteristics,coronal characteristics,posterior echo,color blood flow,resistance index,and the presence or absence of axillary lymph node metastasis(P>0.05).However,the differ-ences in the presence or absence of calcification and elasticity scores in the lesions were statistically significant(P<0.05);the differences between the different molecular types of breast cancer in different pathological types were statistically significant(χ^(2)=14.549,P=0.002).Conclusion Different molecular types of breast cancer in young women have different ultrasound signs.Its molecular classification can provide certain help,which is conducive to the treatment and prognosis evaluation of breast cancer in young women.
