慢性阻塞性肺疾病患者应用多功能呼吸康复排痰阀的效果观察

Observation on the effect of applying multifunctional respiratory rehabilitation expectoration valve in patients with chronic obstructive pulmonary disease

目的 探讨慢性阻塞性肺疾病(COPD)患者应用多功能呼吸康复排痰阀的效果观察。方法 选取2021年7月—2022年6月在我院呼吸与危重症医学科就诊的60例COPD患者,将患者分为A组与B组,每组30例,进行交叉对照研究。每个研究阶段为期4周,A组:第一研究阶段应用多功能呼吸康复排痰阀,第二研究阶段应用缩唇呼吸。B组:两研究阶段的顺序与A组相反。每个阶段后有2周的洗脱期。比较多功能呼吸康复排痰阀与缩唇呼吸两种训练方法对两组患者COPD评估测试(CAT)评分、圣乔治呼吸问卷(SGRQ)评分、博格(Borg)评分、咳嗽咳痰评分。结果 呼吸康复排痰阀与缩唇呼吸两种干预方法对CAT评分、SGRQ评分、Borg评分和咳嗽咳痰评分差异有统计学意义(P<0.05);两个不同试验阶段对CAT评分、SGRQ评分、Borg评分和咳嗽咳痰评分差异有统计学意义(P<0.05)。结论 COPD患者应用多功能呼吸康复排痰阀训练,可促进呼吸道分泌物清除,保持呼吸道通畅,有利于控制肺部感染,缓解呼吸道症状,提升运动耐受能力及活动能力,提高患者的生活质量,改善患者结局。

Objective To observe the effect of applying multifunctional respiratory rehabilitation expectoration valve in patients with chronic obstructive pulmonary disease(COPD).Methods To select 60 COPD patients who were treated in the department of respiratory and critical care medicine of our hospital from July 2021 to June 2022,the patients were divided into group A and group B,with 30 cases in each group to conduct a crosscontrol study.Each research phase lasts for 4 weeks.In group A,the multifunctional respiratory rehabilitation expectoration valve was used in the first research phase,and pursed lip breathing was used in the second research phase.The order of the two study phases was opposite to that of group A in the group B.There was a 2-week washout period after each session.The COPD assessment test(CAT) scores,ST.George's respiratory questionaire(SGRQ) score,Borg score,and cough and sputum scores of the two groups of patients were compared between the two training methods of multifunctional respiratory rehabilitation expectoration valve and pursed lip breathing.Results The two intervention methods of respiratory rehabilitation expectoration valve and pursed-lip breathing had statistically significant differences in CAT scores,SGRQ score,Borg score,and cough and sputum scores(P<0.05);the two different trial stages had statistically significant differences in CAT scores,SGRQ score,Borg score,and cough and sputum scores(P<0.05).Conclusion The application of multifunctional respiratory rehabilitation sputum valve training in patients with COPD can promote the clearance of respiratory secretions,keep the respiratory tract open,help control lung infection,relieve respiratory symptoms,improve exercise tolerance and activity ability,and improve the patient's quality of life and improved patient outcomes.
