卒中救治定点医院静脉溶栓现状调查

Investigation on the current situation of intravenous thrombolysis in designated hospitals for stroke treatment

目的 了解贵阳市卒中救治定点医院急性缺血性脑卒中救治能力现状。方法 以2017年贵州省卫生健康委员会发布的贵阳市脑卒中“溶栓地图”13所卒中救治定点医院为调查对象。采用自制问卷,从“医院基本情况、卒中救治流程、信息管理、卒中救治人员管理”4个方面进行调查。结果 13所卒中救治定点医院中年溶栓量≥100例的占23.08%,小于10例的占38.46%;急性缺血性脑卒中静脉溶栓地点主要在神经内科病房,占61.54%,DNT达标的医院占61.54%;69.23%的医院设有急性卒中病例数据库并能够进行急性卒中病例登记;69.33%的医院至少每3个月召开卒中小组会议,定期参加培训的急救人员占76.92%。结论 贵阳市卒中救治定点医院的静脉溶栓现况不容乐观,卒中急诊就诊流程、信息管理建设、卒中救治人员的培训管理工作需进一步完善。患者及家属的溶栓决策情况需要改善。

Objective To understand the current situation of acute ischemic stroke treatment capacity in designated hospitals for stroke treatment in Guiyang.Methods We conducted a survey on 13 designated hospitals for stroke treatment in Guiyang,based on the "thrombolysis map" released by the Guizhou Provincial Health Commission in 2017.A self-made questionnaire was used to investigate the basic situation of the hospital,stroke treatment process,information management,and management of stroke treatment personnel.Results In 13designated stroke hospitals,23.08% of middle-aged patients had a thrombolysis volume of ≥ 100 cases,while 38.46% had a thrombolysis volume of less than 10 cases;the location of intravenous thrombolysis for acute ischemic stroke is mainly in the neurology ward,accounting for 61.54%,and hospitals that meet DNT standards account for 61.54%;69.23% of hospitals have an acute stroke case database and are able to register acute stroke cases;69.33% of hospitals hold stroke group meetings at least every 3 months,and 76.92% of emergency personnel participate in regular training.Conclusion The current situation of intravenous thrombolysis in designated hospitals for stroke treatment in Guiyang was poor,and the emergency treatment process,information management construction,and training management of stroke rescue personnel need to be further improved.The thrombolysis decision-making of patients and their families needed to be improved.
