摘要
目的探讨经口低温等离子技术在扁桃体癌切除术中的疗效。方法回顾性分析2018年12月至2021年5月就诊于南京鼓楼医院耳鼻咽喉头颈外科就诊和治疗的16例扁桃体癌患者的临床资料。所有患者均行经口低温等离子扁桃体癌切除术,8例患者术后行同步的放疗和化疗,3例仅行放疗,1例仅行化疗,均定期复查。分析术后患者吞咽、呼吸、发声功能等情况及远处转移情况。结果所有患者均成功施行手术,术后均无严重并发症,局部及颈部淋巴结无复发,未见远处转移。均可正常进食,无呛咳,无吞咽困难等;复查喉镜无咽瘘,口咽部疤痕增生,咽腔无狭窄,发音良好。除1例患者化疗后骨髓抑制,右上肢静脉血栓外,其余11例患者均愈后良好,无并发症。结论经口低温等离子切除扁桃体癌术后恢复良好,咽腔结构保留完好,不影响正常功能,值得临床推广。
Objective To investigate the effect of transoral low-temperature plasma technique in tonsil carcinoma resection.Methods The clinical data of 16 patients with tonsil cancer who were treated in the Department of Otolaryngology,Head and Neck Surgery of Nanjing Drum Tower Hospital from December 2018 to May 2021 were retrospectively analyzed.All patients underwent transoral low-temperature plasma resection of tonsil carcinoma.8 patients received simultaneous radiotherapy and chemotherapy,3 patients received radiotherapy only,and 1 patient received chemotherapy only.All patients were reviewed regularly.The swallowing,breathing,vocal function and distant metastasis were analyzed.Results All patients were successfully operated without serious complications,no recurrence of local and cervical lymph nodes,and no distant metastasis.All of them could eat normally without choking or dysphagia.Reexamination laryngoscope showed no pharyngeal fistula,oropharyngeal scar hyperplasia,pharyngeal cavity stenosis and good pronunciation.Except for 1 patient with myelosuppression and right upper limb venous thrombosis after chemotherapy,the other 11 patients recovered well without complications.Conclusion Transoral low-temperature plasma resection of tonsil carcinoma has good recovery,and the structure of pharyngeal cavity is intact,without affecting the normal function,which is worthy of clinical promotion.
