单侧与双侧经皮椎体成形术对老年骨质疏松性胸腰椎压缩性骨折患者腰椎功能及疼痛程度的影响对比分析

Comparative analysis of the effects of unilateral and bilateral percutaneous vertebroplasty on lumbar function and pain in elderly patients with osteoporotic thoracolumbar compression fractures

目的:对比单侧与双侧经皮椎体成形术(PVP)治疗老年骨质疏松性胸腰椎压缩性骨折(OVCF)的临床疗效。方法:2020年6月至2023年6月本院行PVP治疗的70例老年OVCF患者,根据穿刺方法分为单侧组(n=36,单侧穿刺行PVP手术)和双侧组(n=34,双侧穿刺行PVP手术)。比较两组的手术指标。在术前、术后3 m、6 m采用ODI和VAS量表评估两组患者腰椎功能及疼痛程度。统计并发症情况。结果:单侧组手术时间更短、透视次数和骨水泥用量更少(P<0.05)。双侧组术后6 m ODI、VAS评分低于单侧组(P<0.05)。结论:双侧穿刺行PVP治疗OVCF在缓解疼痛及改善腰椎功能方面更优,但单侧穿刺可缩短手术时间,减少透视次数和骨水泥用量。

Objective:To compare the clinical efficacy of unilateral and bilateral percutaneous vertebroplasty(PVP)in the treatment of senile osteoporotic thoracolumbar compression fracture(OVCF).Methods:From June 2020 to June 2023,70 elderly patients with OVCF who underwent PVP treatment in our hospital were divided into unilateral group(n=36,unilateral puncture for PVP surgery)and bilateral group(n=34,bilateral puncture for PVP surgery)according to puncture methods.The surgical indexes of the two groups were compared.ODI and VAS scales were used to evaluate the lumbar function and pain degree of the two groups before surgery,3 months and 6 months after surgery.Complications were counted.Results:The unilateral group had shorter operation time,less fluoroscopy times and less bone cement dosage(P<0.05).The ODI and VAS scores of bilateral group were lower than those of unilateral group 6 months after operation(P<0.05).Conclusion:Bilateral PVP is better in relieving pain and improving lumbar function in the treatment of OVCF,but unilateral puncture can shorten the operation time,reduce the number of fluoroscopy and the amount of bone cement.
