HP联合HD对有机磷农药中毒患者血清ChE、IL-6、hs-CRP水平及凝血功能的影响

Effect of HP combined with HD on serum ChE,IL-6 and hs-CRP levels and coagulation function in patients with organophosphorus pesticide poisoning

目的:探究血液灌流联合血液透析对有机磷农药中毒患者血清胆碱酯酶活性、白介素-6、超敏C反应蛋白及凝血功能的影响。方法:回顾性分析2021年1月至2023年6月本院收治的68例有机磷农药中毒患者临床资料,按治疗方式不同将患者分为对照组32例(行常规治疗)与研究组36例(行血液灌流联合血液透析治疗)。对比两组住院时间、血清胆碱酯酶恢复正常时间、苏醒时间,治疗前后的炎性指标、血功能、肝功能,并统计两组并发症发生情况。结果:研究组住院时间、血清胆碱酯酶恢复正常时间、苏醒时间均比对照组短(P<0.05);治疗后,两组血清胆碱酯酶活性比治疗前高,白介素-6、超敏C反应蛋白均比治疗前低,且研究组优于对照组(P<0.05);治疗后,两组血清活化部分凝血活酶时间、谷丙转氨酶、血肌酐水平比治疗前低,且研究组低于对照组(P<0.05);研究组反跳现象、中间综合征、多器官功能障碍综合征发生率低于对照组(P<0.05)。结论:对有机磷农药中毒患者给予血液灌流联合血液透析治疗可以缩短患者苏醒时间,提高血清胆碱酯酶活性,降低机体炎症反应及并发症发生率。

Objective:To investigate the effect of hemoperfusion combined with hemodialysis on serum cholinesterase activity,interleukin-6 and hypersensitive C-reactive protein levels and coagulation function in patients with organophosphorus pesticide poisoning.Methods:The clinical data of 68 patients with organophosphorus pesticide poisoning who were admitted to our hospital from January 2021 to June 2023 were analyzed retrospectively.According to different treatment methods,the patients were divided into two groups.32 patients in the control group were given conventional treatment and 36 patients in the study group were given HP combined with HD.The length of hospital stays,recovery time of serum cholinesterase,awakening time,inflammatory factors,coagulation function and liver function were compared between the two groups.The incidence rates of complications in the two groups were calculated.Results:The length of hospital stays;recovery time of serum cholinesterase and awakening time of the study group were shorter than those of the control group(P<0.05).After treatment,the activity of serum cholinesterase in the study group was higher than that in the control group,and the levels of interleukin-6 and hypersensitive Creactive protein were lower than those in the control group(P<0.05).After treatment,activated partial thromboplastin time,alanine transaminase and serum creatinine level in study group were lower than those in the control group(P<0.05).The incidence rates of rebound phenomenon,intermediate syndrome and multiple organ dysfunction syndrome in the study group were lower than those in the control group(P<0.05).Conclusion:The combined treatment of hemoperfusion and hemodialysis for patients with organophosphorus pesticide poisoning can shorten their awakening time,increase the activity of serum ChE,and reduce the incidence of inflammatory reactions and complications.
