USP7和DNMT1在乙肝相关性肝癌中的表达及意义

Expression and significance of USP7 and DNMT1 in hepatitis B-associated hepatocellular carcinoma

目的 :研究泛素特异性蛋白酶7(ubiquitin-specific proteinase, USP7)、DNA甲基化转移酶1(DNA methylation transferase 1, DNMT1)在慢性乙型病毒性肝炎(以下简称乙肝)相关性肝癌中的表达及临床意义。方法 :收集乙肝患者17例、乙肝肝硬化患者18例、乙肝相关性肝癌患者30例和健康体检者19例血清,采用ELISA法检测USP7、DNMT1、甲胎蛋白(alpha fetoprotein, AFP)的表达情况,并分析其临床意义。结果:肝癌患者血清AFP、USP7、DNMT1水平较健康体检者、乙肝及肝硬化患者高(P<0.05)。肝癌患者血清中USP7与肿瘤分期、Child-Pugh分级呈正相关(P<0.05),DNMT1与肿瘤分期、肿瘤个数呈正相关(P<0.05)。AFP、USP7、DNMT1均为乙肝相关性肝癌的影响因素(均P<0.05)。血清AFP、USP7、DNMT1水平均在肝细胞癌中有诊断效能(均P<0.05),且联合检测诊断效能高于单一检测。结论 :肝癌患者血清AFP、USP7、DNMT1水平较高,USP7、DNMT1与临床病理特征相关。三项指标联合测定能提高鉴别乙肝相关性肝癌的诊断效能。

Objective:To study the expression and clinical significance of ubiquitin-specific proteinase(USP7)and DNA methylation transferase 1(DNMT1)in hepatitis B-associated hepatocellular carcinoma.Methods:Serum from 17 patients with hepatitis B,18 patients with hepatitis B cirrhosis,30 patients with hepatitis B-associated hepatocellular carcinoma and 19 normal human were collected,and the clinical significance of USP7,DNMT1 and alpha fetoprotein(AFP)was detected by ELISA.Results:The serum levels of AFP,USP7,and DNMT1 were higher in hepatocellular carcinoma patients than in normal subjects,hepatitis B and cirrhosis patients in the three groups(P<0.05).Serum USP7 was positively correlated with tumor stage and Child-Pugh grade in hepatocellular carcinoma patients(P<0.05),and DNMT1 was positively correlated with tumor stage and number of tumors(P<0.05).AFP,USP7,and DNMT1 were all influential factors in hepatitis B-associated hepatocellular carcinoma(all P<0.05).Serum AFP,USP7 and DNMT1 levels all had diagnostic efficacy in hepatocellular carcinoma(P<0.05),and the diagnostic efficacy of the combined test was higher than that of the single test.Conclusion:Serum AFP,USP7 and DNMT1 levels were higher in hepatocellular carcinoma patients,and USP7 and DNMT1 were correlated with clinicopathologic features.The combined determination of the three indexes can improve the diagnostic efficacy in identifying hepatocellular carcinoma.
