唇腭裂对腺样体生长发育影响的研究进展

Effects of cleft lip and palate on the growth and development of adenoids

唇腭裂(cleft lip and palate, CLP)是口颌面部最常见的先天性畸形,腺样体是位于鼻咽腔气道的不规则淋巴组织。腺样体肥大(adenoid hypertrophy, AH)可引起鼻塞、打鼾、口呼吸,甚至出现腺样体面容,影响患者身心健康。近年来多数研究认为CLP影响腺样体生长发育致其肥大,部分AH的CLP患儿需在序列治疗过程中纳入腺样体切除手术,故临床医师应准确评估CLP患儿腺样体生长趋势。本文分别从局部解剖、全身致病机制及治疗方式等方面就CLP影响腺样体生长发育的可能原因进行综述。

Cleft lip and palate(CLP)is the most common congenital malformation of the oral and maxillofacial region.Adenoids are irregular lymphoid tissues that located in the nasopharyngeal airway.Adenoid hypertrophy(AH)can cause nasal congestion,snoring,mouth breathing,and even cause adenoid face which can affects the physical and mental health of patients.For the past few years,most studies have proved that CLP can affect the growth of adenoids and lead to AH.Some CLP patients with AH need to receive the adenoidectomy as a kind of treatment during multidisciplinary management treatment.Therefore,clinicians should accurately evaluate the growth trend of adenoids in children with CLP.This article reviews the factors that may affect the growth and development of adenoids from the aspects of local anatomy,systemic pathogenic mechanism and treatment methods.
