摘要
目的了解语后聋的双侧人工耳蜗使用者(BCI)的噪声下言语识别能力及音乐感知能力。方法收集2017年1月—2021年2月收治的8例语后聋BCI、26例正常听力受试者和40例单侧极重度聋受试者,采用HOUSE耳研所研发的开放式普通话言语识别评估系统,评估不同听觉条件受试者的噪声言语识别阈(SRT)和音乐音符辨识能力、旋律辨识能力。结果8例语后聋BCI双耳佩戴耳蜗的噪声下SRT与单耳佩戴时的差异无统计学意义,均显著高于单侧耳聋组(P<0.01),且单侧耳聋组高于正常听力组(P<0.01)。语后聋BCI双耳佩戴时的音乐音符识别率高于单耳佩戴时,与单侧耳聋组相似。结论语后聋BCI双耳佩戴耳蜗与单耳佩戴时的噪声下言语识别能力相似,均弱于单侧耳聋者及正常听力者。语后聋BCI的音乐感知能力与其既往音乐基础有关,双耳佩戴时优于单耳佩戴时,与单侧耳聋组的音乐感知能力相似,远低于正常听觉人群。
Objective To study the speech recognition and music perception ability of bilateral cochlear implant(BCI)users with postlingual deafness.Method From January 2017 to February 2021,8 bilateral cochlear implant users with postlingual deafness,26 subjects with normal hearing and 40 subjects with unilateral extremely severe deafness were selected to adopt the open Mandarin speech recognition evaluation system developed by HOUSE Ear Research Institute.The Speech recognition threshold(SRT),musical note recognition ability and melodic contour identification(MCI)of subjects with different hearing conditions were evaluated.Results The SRT of bilateral cochlear implant group in noise was significantly higher than that of unilateral hearing loss group(P<0.01),and the SRT of unilateral hearing loss group was higher than that of normal hearing group(P<0.01).The recognition rate of music notes in BCI among post lingual deaf individuals was greater when the device was worn in both ears,similar to the group with unilateral deafness.Conclusions The speech recognition ability of unilateral deafness under noise is worse than that of normal hearing.The speech recognition ability of BCI in noise is similar between unilateral CI and bilateral CI and worse than that of unilateral deafness group and normal hearing group.The capacity of BCI to detect music in postlingual deafness is dependent on its prior musical background.Wearing BCI with both ears is more advantageous than wearing it with one ear,and its music recognition ability is comparable to that of the unilateral deafness group,though it is much lower than that of the normal hearing population.
