帕金森病患者的常规脑电图与多导睡眠图研究

摘要 目的探讨帕金森病(PD)患者的脑电图(EEG)指标变化与多导睡眠图(PSG)的睡眠参数的各项指标特征。方法选取2021年2月至2023年4月郑州市第三人民医院神经内科收治的50例PD患者作为研究对象(PD组),同期在本院作体检的50例健康体检者作为对照组。记录两组受试者的EEG和PSG的睡眠监测数据,对比两组受试者EEG的波型特点、EEG异常情况及睡眠参数各项指标等,并分析PD患者EEG改变与睡眠障碍的相关性。结果PD组患者的EEG异常率为88.0%,对照组异常率为22.0%,差异有统计学意义(P<0.05)。PD组患者的θ波(66.0%)、δ波(30.0%)及β波(24.0%)出现率均高于对照组。PD组患者的氧减指数高于对照组,总睡眠时间少于对照组,而快速眼动睡眠潜伏期长于对照组,觉醒次数多于对照组,睡眠效率低于对照组,差异均有统计学意义(P<0.05)。Spearman相关性分析结果表明,PD患者EEG的θ波、β波及δ波与觉醒次数、氧减指数及快速眼动睡眠潜伏期呈正相关,与总睡眠时间及睡眠效率呈负相关。结论PD患者的EEG指标与PSG的睡眠参数各项指标之间存在一定的相关性。与健康体检者比较,PD患者的脑电波频率较高,波幅较低,并且存在相位错乱的特点。 Objective To analyze the changes in EEG indicators and polysomnography(PSG)sleep parameter characteristics in patients with Parkinson's disease(PD).Methods Fifty PD patients admitted to the Department of Neurology,Zhengzhou Third People's Hospital from February 2021 to April 2023 were selected as study subjects(PD group),and 50 healthy individuals who underwent physical examinations in the hospital during the same period were selected as control group.The conventional EEG data and PSG sleep monitoring data of two groups were recorded.EEG wave characteristics,abnormal EEG situations and sleep parameter indicators,etc.were compared between two groups.The correlation between EEG changes and sleep disorders was also analyzed.Results The abnormal EEG rate of PD group was significantly higher than that of control group(88.0%vs 22.0%,P<0.05).The occurrence rates ofθwave(66.0%),δwave(30.0%)andβwave(24.0%)were higher in PD group than those of control group.Oxygen desaturation index was higher in PD group than that in control group.Total sleep time was less in PD group than that in control group,while rapid eye movement sleep latency was longer in PD group than that in control group.The number of wake after sleep onset was more in PD group than that in control group.Sleep efficiency was lower in PD group than that in control group.These differences were statistically significant(P<0.05).Spearman correlation analysis showed thatθwaves,βwaves andδwaves were positively correlated with the number of wake after sleep onset and rapid eye movement sleep latency,and negatively correlated with total sleep time and sleep efficiency.Conclusion There is a certain correlation between the conventional EEG indicators and PSG sleep parameter indicators in patients with PD.The patients with PD have high EEG wave frequencies,low amplitudes and phase disturbances when compared with healthy individuals.
