CT影像诊断支气管扩张症的临床价值

Clinical value of CT imaging in the diagnosis of bronchiectasis

目的:分析CT影像诊断支气管扩张症的临床价值。方法:选取2020年1月—2023年10月在菏泽市第三人民医院就诊的360例疑似支气管扩张症患者,所有患者均进行常规CT检查、多层螺旋CT检查,观察两种检查方法的诊断价值。结果:支气管活检共确诊支气管扩张症315例,常规CT检出真阳性例数225例,多层螺旋CT检查的真阳性例数为301例。多层螺旋CT检查的灵敏度、特异度、准确率、阳性预测值、阴性预测值均高于常规CT检查(P<0.05)。多层螺旋CT检查的肺段总检出准确率95.56%明显高于常规CT检查的71.43%(P<0.05),且柱状扩张型的比例最高,其次为静脉曲张型。结论:多层螺旋CT检查具有较高的诊断效能,对于筛查诊断支气管扩张症的效果显著,并可有效检出肺段扩张情况。

Objective To analyze the clinical value of CT imaging in the diagnosis of bronchiectasis.Methods A total of 360 patients with suspected bronchiectasis treated in the Third People's Hospital of Heze City from January 2020 to October 2023 were selected.All patients underwent routine CT examination and multi-slice spiral CT examination to observe the diagnostic value of the two examination methods.Results A total of 315 cases of bronchiectasis were confirmed by bronchial biopsy,225 cases of true positive by conventional CT and 301 cases of true positive by multi-slice spiral CT.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of multi-slice spiral CT were higher than those of conventional CT(P<0.05).The total detection rate of lung segments by multi-slice spiral CT was 95.56%,higher than that by conventional CT(71.43%)(P<0.05).The proportion of columnar dilated type was the highest,followed by varicose type.Conclusion Multi-slice spiral CT(MSCT)has a high diagnostic efficacy,which is significant in the screening and diagnosis of bronchiectasis,and can effectively detect the expansion of lung segments.
