浮针联合关节镜手术治疗痛风性膝关节炎的临床研究

Clinical Study of Floating Needle Combined with Arthroscopic Surgery in the Treatment of Gouty Knee Arthritis

目的:研究关节镜手术配合浮针治疗痛风性膝关节炎的临床疗效。方法:选取2017年1月—2022年1月广州中医药大学附属佛山市中医院三水医院收治的68例单侧痛风性膝关节炎患者作为研究对象,随机分为观察组和对照组,每组各34例。对照组在中成药物治疗的基础上采用关节镜手术进行治疗,观察组在对照组治疗的基础上配合浮针疗法疗法。比较两组患者的疗效、血清学指标和关节功能评分。结果:两组患者临床治疗总有效率相比较,差异无统计学意义(χ^(2)=0.731,P>0.05);治疗后,两组患者血清尿酸(UA)、C反应蛋白(CRP)、红细胞沉降率(ESR)和白细胞介素-6(IL-6)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(t=9.100、3.094、2.354、4.372,P<0.05)。治疗后,两组患者视觉模拟(VAS)评分、Lysholm膝关节评分均高于治疗前,且观察组高于对照组,差异有统计学意义(t=1.578、3.014、1.062、2.451,P<0.05;t=1.623、2.011、1.132、2.017,P<0.05)。结论:关节镜手术配合浮针治疗可以明显缓解患者疼痛,利于关节功能的恢复,提高患者就诊治疗体验感及满意度。

Objective:To study the clinical efficacy of arthroscopic surgery with floating needle in the treatment of gouty knee osteoarthritis.Methods:68 cases of unilateral gouty knee arthritis patients admitted to the hospital from January 2017 to January 2022 were selected as the study subjects and randomly divided into two groups,with 34 cases in each group.The control group was treated with arthroscopic surgery on the basis of the treatment of Chinese patent medicines,and the observation group was treated with floating needle therapy therapy on the basis of the treatment of the control group.The serologic indexes and joint function scores of the two groups were compared.Results:There was no statistically significant difference in the total effective rate of clinical treatment between the two groups(χ^(2)=0.731,P>0.05).After treatment,the levels of serum uric acid(UA),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and interleukin-6(IL-6)were lower than those before treatment in both groups,and the observation group was lower than that of the control group,and the difference was statistically significant(t=9.100,3.094,2.354,4.372,P<0.05).After treatment,the visual analog scores(VAS scores)and Lysholm knee scores of patients in both groups were higher than those before treatment,and the observation group was higher than the control group,and the difference was statistically significant(t=1.578,3.014,1.062,2.451,P<0.05;t=1.623,2.011,1.132,2.017,P<0.05).Conclusion:Arthroscopic surgery with floating needle treatment can significantly relieve patients’pain,facilitate the recovery of joint function,and improve patients’experience and satisfaction with the treatment.
