老年2型糖尿病伴冠状动脉粥样硬化性心脏病心绞痛的影响因素分析

Analysis of Influencing Factors of Type 2 Diabetes Mellitus with Coronary Atherosclerotic Heart Disease and Angina Pectoris in Elderly Patients

目的 探讨老年2型糖尿病(type 2 diabetes mellitus,T2DM)伴冠状动脉粥样硬化性心脏病心绞痛(coronary atherosclerotic heart disease angina pectoris,CHDAP)的影响因素。方法 回顾性选取2022年1月—2023年4月厦门市康复医院心内科收治的共计148例老年T2DM患者的临床资料,根据是否合并CHDAP将其分成单纯T2DM组(97例)与合并CHDAP组(51例),分析两组临床特征,并进行Logistic多因素回归分析。结果 与单纯T2DM组相比,合并CHDAP组年龄≥70岁、体质指数(body mass index,BMI)≥25 kg/m^(2)、糖尿病病程≥6年、合并高血压、高密度脂蛋白胆固醇(high density liptein cholesterol,HDL-C)<1.0 mmol/L、糖化血红蛋白(glycated hemoglobin,HbA1c)≥8.5%占比更高,差异有统计学意义(P<0.05);Logistic多因素分析结果显示,年龄(OR=2.164)、BMI(OR=2.729)、糖尿病病程(OR=3.045)、合并高血压(OR=3.382)、HDL-C(OR=4.256)、HbA1c(OR=3.197)是老年T2DM伴CHDAP的独立危险因素(P<0.05)。结论 年龄、BMI、糖尿病病程、合并高血压、HDL-C、HbA1c是老年T2DM伴CHDAP的危险因素,应采取针对性预防措施,降低CHDAP发生率。

Objective To investigate the influencing factors of type 2 diabetes mellitus(T2DM)with coronary athero-sclerotic heart disease angina pectoris(CHDAP)in the elderly.Methods The clinical data of 148 elderly T2DM pa-tients admitted to the department of cardiology of Xiamen Rehabilitation Hospital from January 2022 to April 2023 were retrospectively selected,and they were divided into T2DM group alone(97 cases)and CHDAP group combined(51 cases)according to whether CHDAP was present or not.The clinical features of the two groups were analyzed,and Logistic multivariate regression analysis was performed.Results Compared with T2DM group,the age≥70 years old,body mass index(BMI)≥25 kg/m^(2),duration of diabetes≥6 years,hypertension,high density liptein cholesterol(HDL-C)<1.0 mmol/L,and glycated hemoglobin(HbA1c)≥8.5%were higher in CHDAP group,and the difference was statis-tically significant(P<0.05).Logistic multivariate analysis results showed that age(OR=2.164),BMI(OR=2.729),dura-tion of diabetes(OR=3.045),combined hypertension(OR=3.382),HDL-C(OR=4.256),HbA1c(OR=3.197)were inde-pendent risk factors for T2DM with CHDAP in the elderly(P<0.05).Conclusion Age,BMI,duration of diabetes,hy-pertension,HDL-C and HbA1c are the risk factors for CHDAP in elderly T2DM patients.Targeted preventive mea-sures should be taken to reduce the incidence of CHDAP.
