CT肺动脉造影参数诊断肺动脉高压的价值

The Value of CT Pulmonary Angiography Parameters in Diagnosing Pulmonary Arterial Hypertension

目的:探讨CT肺动脉造影(CTA)参数诊断肺动脉高压(PAH)的价值。方法:选取2020年6月至2023年6月本院收治的35例疑似PAH患者为对象,根据右心导管术结果将患者分为PAH组(n=30)和非PAH组(n=5)。PAH患者根据mPAP分为轻度组(n=4)、中度组(n=6)、重度组(n=20)。均接受CTA检查,分析CTA参数与肺动脉压力的关系及其诊断PAH的价值。结果:PAH组dPA、rPA高于非PAH组(P<0.05)。dPA、rPA、mPAP在轻度组、中度组、重度组中依次升高(P<0.05)。相关性分析显示,dPA、rPA与mPAP均呈正相关(P<0.05)。dPA、rPA、dPA+rPA诊断PAH的AUC分别为0.777(95%CI:0.604-0.899)、0.807(95%CI:0.638-0.920)、0.960(95%CI:0.833-0.998)。dPA+rPA诊断PAH的AUC高于dPA、rPA(P<0.05)。结论:PAH患者CTA参数如dPA、rPA值升高,与肺动脉压力呈线性正相关,可用于PAH的诊断。

Objective:Exploring the Value of CT Pulmonary Angiography(CTA)Parameters in Diagnosing Pulmonary Arterial Hypertension(PAH).Methods:Thirty-five suspected pulmonary arterial hypertension(PAH)patients admitted to our hospital from June 2020 to June 2023 were selected as the subjects.Based on the results of right heart catheterization,patients were divided into the PAH group(n=30)and non-PAH group(n=5).Among the PAH patients,they were categorized into mild group(n=4),moderate group(n=6),and severe group(n=20)based on mean pulmonary arterial pressure(mPAP).All patients underwent CTA examination,and the relationship between CTA parameters and pulmonary artery pressure was analyzed,evaluating their diagnostic value for PAH.Results:The dPA and rPA in the PAH group were higher than those in the non-PAH group(P<O.05).The dPA,rPA,and mPAP increased successively in the mild group,moderate group,and severe group(P<0.05).The correlation analysis indicated a positive correlation between dPA,rPA,and mPAP(P<0.05).The AUC for diagnosing PAH based on dPA,rPA,and dPA+rPA were 0.777(95%CI:0.604-0.899),0.807(95%CI:0.638-0.920),and 0.960(95%CI:0.833-0.998),respectively.The AUC for diagnosing PAH based on dPA+rPA was higher than that of dPA and rPA(P<0.05).Conclusion:Elevated CTA parameters such as dPA and rPA values in PAH patients show a linear positive correlation with pulmonary artery pressure,indicating their potential for diagnosing PAH.
