摘要
胜任力为导向的医学教育(competency-based medical education,CBME)是近年来我国医学改革的主要发展趋势,但其在实施上存在一定的困难,本文以内分泌学科为例,基于置信职业行为(EPAs)构建了依年资分层递进、适合住院医师在内分泌科轮转期间的培训内容和考核模式,经过问卷调查询问专家意见后形成了16项临床行为,每项内容包括对应的核心能力及其在不同阶段预期的置信等级。为确保培训和评估的有效性和准确性,本研究团队加强了对指导教师的培训,同时根据不同阶段学员的知识技能水平选择合适的教学策略和评估工具,并从学员和教师对课程的反馈来评估教学效果,提供了基于EPAs的内分泌住院医师规范化培训内容的模板。
Competency-based medical education(CBME)is the main development trend of China’s medical reform in recent years,but it has certain problems in its implementation.Taking endocrinology as an example,this paper constructs the training content and assessment mode of hierarchical and suitable for residents during the rotation of endocrinology based on EPAs(entrustable professional activities,EPAs).After questionnaires and expert opinions,16 clinical behaviors were formed,each covering the corresponding core competencies and their expected entrutable levels at different stages.To ensure the effectiveness and accuracy of training and assessment,strengthen the training of instructors is necessary.Meanwhile,appropriate teaching strategies and assessment tools are selected according to the knowledge and skill level of students at different stages. And also,from the feedback of students and teachers on the course to evaluate the teaching effect,provide a template for EPAs-based standardized training content for endocrinology residents.
