舍曲林联合认知行为疗法对室性心律失常射频消融术后患者心率变异性、心理弹性及睡眠状态的影响

Effects of Sertraline combined with cognitive behavioral therapy on heart rate variability,mental resilience and sleep status in patients with ventricular arrhythmia after radiofrequency ablation

目的探讨舍曲林联合认知行为疗法在室性心律失常射频消融术后患者中的应用。方法回顾性选取我院2020年7月~2023年3月102例室性心律失常射频消融术后患者为研究对象,随机数字表法分为药物组和认知组,各51例。两组均给予抗凝、抗心律失常等对症支持治疗,药物组予舍曲林治疗,认知组在药物组基础上予认知行为疗法,持续治疗8周,对比两组心率变异性、心理弹性及睡眠状态。结果认知组治疗8周后RR间期平均值标准差(standard deviation average of NN intervals,SDANN)、相邻RR间期差值均方根(the root mean square of successive R-R interval,RMSSD)、窦性RR间期标准差(standard deviation of NN intervals,SDNN)水平高于药物组(P<0.05)。认知组治疗8周后坚韧、乐观、自强评分高于药物组(P<0.05)。认知组治疗8周后睡眠时间、睡眠效率、入睡时间评分低于药物组(P<0.05)。结论舍曲林联合认知行为疗法可改善室性心律失常射频消融术后患者心理弹性,调节心率变异性,调控睡眠状态。

Objective To investigate the application of sertraline combined with cognitive behavioral therapy in patients with ventricular arrhythmia after radiofrequency ablation.Methods A total of 102 patients with ventricular arrhythmia after radiofrequency ablation in our hospital from July 2020 to March 2023 were retrospectively selected as the study objects,and were divided into drug group and cognitive group by random number table method,with 51 cases in each group.The two groups were given symptomatic supportive treatment such as anticoagulation and antiarrhythmic treatment,the drug group was given sertraline treatment,and the cognitive group was given cognitive behavioral therapy based on the drug group for 8 weeks.The heart rate variability,mental elasticity and sleep status of the two groups were compared.Results Standard deviation average of NN intervals after 8 weeks of cognitive treatment(SDANN),the root mean square of successive R-R interval(RMSSD),standard deviation of NN intervals,The level of SDNN was higher than that of drug group(P<0.05).After 8 weeks of treatment,the scores of tenacity,optimism and self-reliance in cognitive group were higher than those in drug group(P<0.05).After 8 weeks of treatment,the scores of sleep time,sleep efficiency and sleep time in cognitive group were lower than those in drug group(P<0.05).Conclusion Sertraline combined with cognitive behavioral therapy can improve mental resilience,regulate heart rate variability and regulate sleep status of patients with ventricular arrhythmia after radiofrequency ablation.
