2019—2022年我院住院患者使用β-内酰胺酶抑制剂复合制剂的用药情况分析

Analysis of the Use of β-lactamase Inhibitor Compound Preparations by Inpatients in Our Hospital from 2019 to 2022

目的:了解我院住院患者使用β-内酰胺酶抑制剂复合制剂的情况、变化趋势,为抗菌药物的临床应用及科学管理提供参考。方法:通过院内His平台系统导出我院2019年1月—2022年12月住院药房抗菌药物的应用情况,对药品数量、药品总销售额、用药频度(DDD数)、日均药费(DDC)、序号比等信息按年进行数据处理和分析。并抽取2021—2022年共1319份抗菌药物病历医嘱进行点评。结果:阿莫西林克拉维酸钾DDDs排序第1位,序号比>1,价格与使用频率同步性好;注射用哌拉西林钠他唑巴坦钠(4∶1)使用金额稳居第一,但序号比<1,患者经济负担较重;注射用头孢哌酮钠舒巴坦钠(2∶1)DDDs排序居第2位,序号比≥1,同步性较好。β-内酰胺酶抑制剂复合制剂不合理使用病历共11份。结论:2019年1月—2022年12月我院β-内酰胺酶抑制剂复方制剂临床应用基本合理,但也存在个别违规使用抗菌药物的情况,今后药剂科还应继续强化监督管理,加强抗菌药物的合理使用。

Objective:To understand the situation and change trend ofβ-lactamase inhibitor compound preparation used by inpatients in our hospital,and to provide reference for clinical application and scientific management of antibiotics.Methods:The application of antibiotics in inpatient pharmacies of our hospital from January 2019 to December 2022 was exported through his platform system,and the data of drug quantity,total drug sales,drug frequency(DDD number),average daily drug cost(DDC),serial number ratio and other information were processed and analyzed annually.A total of 1319 medical records of antibiotics from 2021 to 2022 were selected for comments.Results:Amoxicillin and clavulanate potassium DDDs ranked the first place,and the sequence number ratio was>1.The consumption amount of piperacillin sodium and tazobactam sodium for injection(4∶1)ranked the first,but the serial number ratio was<1,and the economic burden of patients was heavier.The DDDs of cefoperazone sodium and sulbactam sodium for injection(2∶1)ranked the second,the sequence number ratio was≥1,and the synchronization was good.There were 11 cases of irrational use ofβ-lactamase inhibitor compound preparation.Conclusion:The clinical application ofβ-lactamase inhibitor compound preparations in our hospital from January 2019 to December 2022 is basically reasonable,but there are also individual cases of irregular use of antibiotics.In the future,the pharmacy department should continue to strengthen supervision and management and strengthen the rational use of antibiotics.
