心肌梗死患者心电图fQRS与冠脉病变范围及程度的相关性及预测价值 被引量:1

Correlation and Predictive Value Electrocardiographic fQRS with the Scope and Degree of Coronary Artery Lesions in Patients with Myocardial Infarction

摘要 目的:分析心肌梗死(AMI)患者心电图fQRS与冠脉病变情况的相关性及预测价值。方法:选择2020年2月—2022年2月我院AMI患者80例,根据心电图结果分为非fQRS组、fQRS组,收集患者一般资料,比较两组冠脉病变范围及程度指标,Pearson相关性分析fQRS与冠脉病变指标相关性,COX风险模型分析AMI患者心血管事件因素。结果:fQRS组高血压、糖尿病、心血管事件、病变范围(双多支)、狭窄程度(中重度狭窄)比例及左室舒张末期内径(LVEDd)、Gensini评分高于非fQRS组,左室射血分数(LVEF)低于非fQRS组(P<0.05)。Pearson相关性分析显示,高血压、糖尿病、心血管事件、LVEDd、病变范围、狭窄程度、Gensini评分与fQRS呈正相关,LVEF与fQRS呈负相关(P<0.05)。COX风险模型显示,高血压、糖尿病、LVEDd、病变范围(双多支)、狭窄程度(中重度狭窄)、Gensini评分、fQRS均为AMI患者心血管事件危险因素(P<0.05),LVEF为保护因素(P<0.05)。结论:AMI患者多出现心电图fQRS波,fQRS波与冠脉病变范围及程度存在明显相关性,是AMI患者发生不良心血管事件独立危险预测因子,具有一定应用价值。 Objective:To analyze the correlation and predictive value of electrocardiographic fQRS with coronary lesion status in patients with myocardial infarction(AMI).Methods:A total of 80 patients with AMI in our hospital from February 2020 to February 2022 were selected and divided into non-fQRS group and fQRS group according to the results of electrocardiogram.The general data of patients were collected.The range and degree of coronary artery lesions were compared between the two groups.Pearson correlation analysis was used to analyze the correlation between fQRS and coronary artery lesions.COX risk model was used to analyze the factors of cardiovascular events in AMI patients.Results:The proportion of hypertension,diabetes mellitus,cardiovascular events,lesion extent(double multiple branches),stenosis degree(moderate to severe stenosis)and left ventricular end-diastolic internal diameter(LVEDd)and Gensini score were higher in the fQRS group than in the non-fQRS group,and the left ventricular ejection fraction(LVEF)was lower than in the non-fQRS group(P<0.05).Pearson correlation analysis showed that hypertension,diabetes,cardiovascular events,LVEDd,lesion extent,stenosis,and Gensini score were positively correlated with fQRS,and LVEF was negatively correlated with fQRS(P<0.05).The COX risk model showed that hypertension,diabetes mellitus,LVEDd,lesion extent(double multiple branches),stenosis degree(moderate to severe stenosis),Gensini score,and fQRS were all risk factors for cardiovascular events in AMI patients(P<0.05),and LVEF was a protective factor(P<0.05).Conclusion:Patients with AMI mostly present with ECG fQRS waves.fQRS waves are significantly correlated with the extent and degree of coronary lesions and are independent risk predictors of adverse cardiovascular events in patients with AMI,and have some application value.
