经皮穴位电刺激改善全麻下脊柱手术患者苏醒时间临床观察

Clinical study on improvement of awakening time of patients undergoing spinal surgery under general anesthesia by percutaneous acupoint electrical stimulation

目的研究观察经皮穴位电刺激(TEAS)对全身麻醉下脊柱手术患者苏醒时间的影响。方法选取93例脊柱手术患者,随机分为3组,每组31例。T组(经皮穴位电刺激组)术前行TEAS(足三里、三阴交)治疗20 min,停用七氟烷后行TEAS(内关、曲池)30 min;N组(经皮非穴位电刺激组)刺激点选择T组4个穴位向外侧旁开2 cm的非穴位处,刺激时间同上;C组为空白对照组。比较3组患者治疗后情况。结果T组治疗后第1次呼之睁眼时间、拔管时间、麻醉观察停留时间、各项症状评分均优于其他2组(P<0.05)。结论对全身麻醉下脊柱手术患者行TEAS(足三里、三阴交、内关、曲池),可缩短患者术后苏醒时间,改善苏醒质量,减轻术后疼痛,值得临床推广使用。

Objective To observe percutaneous acupoints electrical stimulation in improving recovery time in patients undergoing spinal surgery.Methodss Ninety-three spinal surgery patients were randomly divided into percutaneous acupoint electrical stimulation group(T group),percutaneous non-acupoint electrical stimulation group(N group)and control group(C group),with 31l patients in each group.Group T took TEAS(Zusanli(ST36)and Sanyinjiao(SP6))for 20 minutes before surgery,and took TEAS(Neiguan(PC6)and Quchi(LIl1))for 30 minutes after stopping sevoflurane.The stimulation time of group N was the same,and the stimulation points of the four acupoints of group T were selected at the non-acupoints 2cm outside.The control group(group C)was a blank control group and was not treated.The general data,first call eye opening time,extubation time,PACU residence time,postoperative VAS,Steward awakening score 15min after extubation and Ramsay sedation score of the three groups were compared.Results The first call time,extubation time,PACU residence time and postoperative VAS in group T were all lower than those in the other two groups,and the difference was statistically significant(P<0.05).Conclusion TEAS(Taking Zusanli(ST36),Sanyinjiao(SP6),Neiguan(PC6)and Quchi(LIl1))for spinal surgery patients under general anesthesia can shorten the postoperative recovery time,reduce postoperative analgesia and improve the recovery quality of patients,and can be widely used in clinic.
