Chlamydia trachomatis infection rate in different anatomical sites among men who have sex with men in China

目的 了解我国MSM不同解剖部位的沙眼衣原体感染现状。方法 检索2013年1月1日至2023年6月1日公开发表在中国知网、万方数据知识服务平台、维普中文期刊服务平台、Pubmed上的相关文献。使用R 4.2.1软件的meta分析软件包(metaprop、metainf、metareg、metabias)进行Meta分析。结果 经文献检索共获得英文文献87篇,中文文献821篇,对符合纳入排除标准的21篇文献进行分析。泌尿生殖道CT合并感染率为5.89%(95%CI:4.91%~6.96%),直肠CT合并感染率为12.69%(95%CI:8.68%~17.33%),口咽部CT合并感染率为0.90%(95%CI:0.41%~1.55%),双解剖部位CT合并感染率(泌尿生殖道/直肠标本中任一标本检测呈阳性)为22.31%(95%CI:14.53%~31.18%),三解剖部位CT合并感染率(泌尿生殖道/直肠/口咽部标本中任一标本检测呈阳性)为15.55%(95%CI:12.80%~18.50%)。样本来源、调查启动年份、调查开展地区是异质性的主要来源(P<0.10)。结论 我国MSM CT感染率较高,直肠CT感染率高于泌尿生殖道及口咽部,应加强MSM的CT筛查,实现早诊早治。

Objective To investigate the prevalence of chlamydia trachomatis(CT)infection in various anatomical sites among men who have sex with men(MSM)in China.Methods The literatures were obtained from CNKI,Wanfang Data Knowledge Service Platform,VIP Chinese Journal Service Platform,and PubMed from January 2013 to May 2023.A meta-analysis was performed using the meta-analysis package of R 4.2.1 software(metaprop,metainf,metareg,and metabias).Results A total of 87 English and 821 Chinese literatures were collected,from which 21 literature works were analyzed as they met the inclusion and exclusion criteria.The CT infection rates in the urogenital tract,anorectum,and oropharynx were 5.89%(95%CI:4.91%-6.96%),12.69%(95%CI:8.68%-17.33%)and 0.90%(95%CI:0.41%-1.55%),respectively.The combined CT infection rate in two anatomical sites(defined as positive in any specimen of the urogenital tract or anorectum)was 22.31%(95%Cl:14.53%-31.18%),whereas in three anatomical sites(defined as positive in any specimen of the urogenital tract,anorectum or oropharynx specimen)was 15.55%(95%CI:12.80%-18.50%).The factors contributing most significantly to heterogeneity were source of specimen,initiation time,and investigation regions(P<O.10).Conclusions The prevalence of CT infection among MSM in China was high.The anorectum CT infection rate was found to be higher than that in the urogenital tract and oropharynx.It is recommended that stronger CT screening measures should be implemented for MSM,in order to diagnose and treat CT infections early.
