摘要
目的:探究结核分枝杆菌特异性抗原ESAT-6、CFP-10在肺结核组织中的表达。方法:收集经病理学诊断的19例石蜡包埋肺结核组织标本,以20例正常肺组织(肺癌切除标本正常组织断端)为阴性对照,采用免疫组化检测ESAT-6及CFP-10,并与抗酸染色进行对比。结果:ESAT-6及CFP-10的阳性信号主要分布在结核病灶的坏死区域及其周围的巨噬细胞和多核巨细胞中,其分布与抗酸杆菌有关,范围更广。免疫组化检测ESAT-6的敏感度、特异度分别为73.68%、90.00%;CFP-10的敏感度和特异度分别为63.16%和95.00%;抗酸染色敏感度为52.63%,免疫组化的敏感度高于抗酸染色(P<0.05)。结论:采用免疫组化检测ESAT-6及CFP-10在结核诊断中有潜在的应用价值。
Objective:To detect the expression of Mycobacterium tuberculosis specific antigen ESAT-6 and CFP-10 in pulmonary tuberculosis tissue.Methods:A total of 19 paraffin-embedded pulmonary tuberculosis tissue specimens were collected and pathologically diagnosed,and 20 cases of normal lung tissue(normal tissue stumps of lung cancer resection specimens)were negative controls,ESAT-6 and CFP-10 were detected by immunohistochemistry,and compared with acid-fast staining.Results:The positive signals of ESAT-6 and CFP-10 were mainly distributed in the necrotic area of tuberculosis lesions and the surrounding macrophages and multinucleated giant cells,and their distribution was related to acid-fast bacilli and had a wider range.The sensitivity and specificity of immunohistochemical detection of ESAT-6 were 73.68%and 90.00%,respectively,the sensitivity and specificity of CFP-10 were 63.16%and 95.00%,respectively,the sensitivity of acid-fast staining was 52.63%.The sensitivity of immunohistochemistry was higher than that of acid-fast staining(P<0.05).Conclusion:Immunohistochemical detection of ESAT-6 and CFP-10 has potential application value in the diagnosis of tuberculosis.
